神病专科医院门诊药房调剂差错情况分析及其管理对策

摘　　要：目的探讨精神病专科门诊医院药房调剂差错情况及管理对策。方法收集2017年1月-2019年12月的处方资料,统计药房调剂差错情况,对差错原因进行分析便于采取相关的管理对策。结果98652张门诊处方共发现调剂差错60张,涉及患者60例,药房调剂差错类型包括用药剂量差错、用药剂型差错、品种差错、用药方法差错、用药规格差错、药品分发差错,占比分别为11.67%、20.00%、23.33%、13.33%、15.00%、16.67%;年度分析结果显示2018年1~12月门诊西药房调剂差错占比最高,为41.67%(25/60),2019年1~12月门诊西药房调剂差错占比有所降低,为25.00%(15/60);导致处方调剂差错的原因包括药品名称相似、药品包装类似、同名不同规格、发药交代所致差错、其他等,分别占比41.67%、28.33%、16.67%、6.67%、6.67%。结论精神病专科医院门诊药房调剂差错类型较多,导致发生的原因多样,通过分析差错原因便于医院制定一系列防范管理措施,同步加强对门诊西药方人员的教育、药师与患者沟通,并合理利用电子处方系统,同步做好定期自查与审查,并完善给药设备防范措施,进而减少医院门诊药房调剂差错发生。Objective To explore the dispensing errors and Management Countermeasures in the pharmacy of psychiatric outpatient hospital.Methods The prescription data from January 2017 to December 2019 were collected,the dispensing errors in the pharmacy were counted,and the causes of the errors were analyzed to facilitate the adoption of relevant management countermeasures.Results A total of 60 dispensing errors were found in 98652 outpatient prescriptions,involving 60 patients.The types of dispensing errors in the pharmacy included medication dose errors,medication form errors,variety errors,medication method errors,medication specification errors and drug distribution errors,accounting for 11.67%,20.00%,23.33%,13.33%,15.00%and 16.67%respectively.The annual analysis results show that the proportion of dispensing errors in outpatient Western pharmacy is the highest from January 2018 to December 2018,accounting for 41.67%(25/60),and the proportion of dispensing errors in outpatient Western pharmacy has decreased from January 2019 to December 2019,accounting for 25.00%(15/60);the causes of prescription dispensing errors include similar drug names,similar drug packaging,different specifications with the same name,errors caused by drug distribution and explanation,and others,accounting for 41.67%,28.33%,16.67%,6.67%and 6.67%,respectively.Conclusion There are many types of dispensing errors in outpatient pharmacy of psychiatric hospital,resulting in various causes.By analyzing the causes of errors,it is convenient for the hospital to formulate a series of preventive management measures,simultaneously strengthen the education of outpatient western medicine prescription personnel,communication between pharmacists and patients,make rational use of electronic prescription system,do regular self-examination and review at the same time,and improve the preventive measures of drug delivery equipment,so as to reduce the occurrence of dispensing errors in hospital outpatient pharmacy.
