摘 要:目的 了解儿童患者医院感染的细菌流行情况及主要病原菌金黄色葡萄球菌药敏结果 方法 对2018年1月—2021年12月儿童患者送检的细菌培养标本,经涂片判断合格后,使用Phoneix100全自动微生物鉴定仪进行菌株鉴定,K-B法进行药敏实验,WHONET 5.6软件回顾性分析。结果 以痰液标本检出的细菌居于首位,占45.6%,其次为血液(15.9%)、分泌物(9.6%)、尿液(7.7%)。金黄色葡萄球菌的检出率最高,为21.1%,其次为大肠埃希菌(19.0%)、肺炎链球菌(16.6%)、肺炎克雷伯菌(13.3%)。MRSA检出率为35.1%(68/194),D实验阳性率为51.0%(99/194)。对不同抗菌药物的耐药率,青霉素高达94.5%,红霉素为73.2%,万古霉素、替考拉宁和利奈唑胺均为0.0%。结论 重视儿童患者感染细菌的流行趋势,主要检出的金黄色葡萄球菌对多种抗菌药物耐药率较高,对万古霉素、替考拉宁等耐药率较低,加强及提升耐药监测能力,预防和控制耐药菌传播,有利于儿童用药安全。Objective To investigate the prevalence of nosocomial infection and drug susceptibility ofStaphylococcus aureusin pediatric patients.Methods Phoneix100 automatic microbial identification instrument was used to identify pathogenic bacteria,K-B drug sensitivity test was performed,and WHONET 5.6 software was used to analyze bacterial culture samples submitted from children patients from January 2018 to December 2021 after they were judged qualified.Results Pathogenic bacteria were mainly distributed in sputum(45.6%),blood(15.9%),secretions(9.6%),urine(7.7%).The infection rate ofStaphylococcus aureuswas the highest(21.1%),followed byEscherichia coli(19.0%),Streptococcus pneumoniae(16.6%),Klebsiella pneumoniae(13.3%).The positive rate of MRSA was 35.1%(68/194),and the positive rate of D test was 51.0%(99/194).The resistance rates of penicillin,erythromycin and vancomycin,teicoplanin and linezolid were 94.5%,73.2%and 0.0%,respectively.Conclusion Attention should be paid to the bacterial prevalence in children.Staphylococcus aureus mainly detected has a high resistance rate to a variety of antibacterial drugs,but a low resistance rate to vancomycin,teicoranin and other drugs.Strengthening and improving the monitoring ability is conducive to preventing and controlling the spread of drug-resistant bacteria,which is beneficial to children.s medication safety.
