威海市HIV-1抗体阳性者WB带型及免疫功能相关性分析

摘　　要：目的 分析2016年—2021年威海市HIV-1抗体阳性者免疫印迹试验(WB)带型及首次CD4+T淋巴细胞的分期特征,了解抗体及免疫细胞之间的关系。方法 免疫印迹试验检测HIV-1抗体带型,四色抗体流式细胞仪计数CD4+T、CD8+T、双阴性T细胞(DNT)及比例。结果 488例HIV-1抗体阳性者WB带型以全带型和次全带型为主要带型,CD4细胞计数、DNT占比在不同条带数分组中差异有统计学意义(P<0.05),条带p55、p17在不同免疫缺陷分级中出现率差异有统计学意义(P<0.05),且在AIDS期出现率最低。来源于疾病预防控制中心和医院的患者年龄与DNT计数及DNT占比均成负相关,医院患者WB条带数与CD4计数成正相关,医院的患者CD8计数与DNT计数成正相关,与DNT占比成负相关,而疾病预防控制中心的患者CD4、CD8计数与DNT计数均成正相关。结论 HIV-1抗体阳性者的WB带型及免疫细胞计数有助于确定不同人群特征的疾病分期,判断机体免疫状态,及时进行抗病毒治疗,并为免疫功能治疗提供科学依据。Objective To analyze the characteristics of the WB band type and the staging of CD4+T lymphocytes in HIV-1positive subjects from 2016 to 2021 in Weihai City,and to understand the relationship between antibodies and immune cells.Methods Western blot assay was used to detect HIV-1 antibody band type,CD4+T,CD8+T,double negative T cells(DNT) and their proportion were counted by flow cytometry.Results The WB bands of 488 HIV-1 positive patients were mainly full-band and subfull-band.There was statistical significance on the differences in CD4 cell count and DNT ratio among groups with different band numbers(P< 0.05).The occurrence rate of p55 and p17 bands was significantly different in different immune deficiency grades,with the differences of statistical significance(P< 0.05),and the occurrence rate was the lowest in AIDS stage.The age of patients from CDC and hospital was negatively correlated with DNT count and proportion.There was a positive correlation between the number of WB bands and CD4 count in hospital patients.CD8 count of patients in hospitals was positively correlated with DNT count,but negatively correlated with DNT proportion.In patients with disease control,CD4 and CD8 counts were positively correlated with DNT counts.Conclusion The WB band pattern and immune cell count of HIV-1 positive individuals are helpful in determining the disease stage characteristic of different populations,judge the immune state of the body,timely antiviral treatment,and provide scientific basis for immune function treatment.
