基于数据挖掘技术分析杨霓芝治疗原发性肾病综合征用药规律

摘　　要：目的:通过对原发性肾病综合征病案的数据挖掘,探讨分析杨霓芝教授治疗原发性肾病综合征的用药经验、规律及组方特点。方法:收集符合标准的门诊病案111例,共307诊次。对病案作文本转化,建立数据档案,通过名医传承平台系统软件对数据进行分析,总结杨霓芝教授用药经验及组方规律。结果:用药统计显示使用频率较高的前17位核心药物分别为甘草、泽兰、黄芪、桃仁、丹参、女贞子、熟地黄、蒲公英、石韦、菟丝子、山茱萸、墨旱莲、白术、白芍、太子参、土茯苓、当归;临床常用中药组合包括山茱萸-菟丝子、覆盆子-金樱子、酸枣仁-合欢皮、黄芪-泽兰-桃仁等。结论:杨霓芝教授辨证治疗原发性肾病综合征,证型总以“气虚血瘀”为大纲,益气活血或兼利水为主要治疗方法,并形成专病的核心药物及药对,用药平和,药少力专。Objective:To explore and analyze Prof.YANG Nizhi’s administration experience,administration rule and prescription characteristics in treating primary nephrotic syndrome through data mining of the medical records of primary nephrotic syndrome.Methods:A total of 111 cases of outpatient medical records were collected,involving 307 visits.Medical records were transformed into texts;data files were established;data were analyzed through the system software of Famous Physicians’ Inheritance Platform,and Prof.YANG’s administration experience and prescription rules were summarized.Results:The statistics of herbs used showed that the top 17 core herbs with high frequency were Radix et Rhizoma Glycyrrhizae,Limonitum,Radix Astragali,Semen Persicae,Radix et Rhizoma Salviae Miltiorrhizae,Fructus Ligustri Lucidi,Radix Rehmanniae Praeparata,Herba Taraxaci,Folium Pyrrosiae,Semen Cuscutae,Fructus Corni,Herba Ecliptae,Rhizoma Atractylodis Macrocephalae,Radix Paeoniae Alba,Radix Pseudostellariae,Rhizoma Smilacis Glabrae and Radix Angelicae Sinensis.Clinical commonly-used Chinese herbal combination included:Fructus Corni-Semen Cuscutae,Fructus Rubi-Fructus Rosae Laevigatae,Semen Ziziphi Spinosae-Cortex Albiziae,and Radix Astragali-Herba Lycopi-Semen Persicae.Conclusion:In the experience of Prof.YANG in differentiating primary nephrotic syndrome,the syndrome type is mainly qi deficiency and blood stasis,and the method of boosting qi and invigorating blood or the method combining the method of promoting urination are the main treatment methods.Core herbs and herbal pairs that particularly treat the disease are obtained,and his prescriptions mainly consist of neutral herbs.Prof.YANG uses herbs concisely but the effects of them are particular.
