三七接骨丸配合空心钉内固定术治疗老年股骨颈骨折临床研究

摘　　要：目的:观察三七接骨丸配合空心钉内固定术治疗老年股骨颈骨折的临床疗效。方法:选取112例老年股骨颈骨折患者,按随机数字表法分为研究组与对照组各56例,其中研究组剔除6例,最终纳入50例。2组均接受空心钉内固定术治疗,对照组术后给予常规止痛、抗感染、预防骨质疏松等治疗,研究组在对照组基础上给予三七接骨丸治疗。比较2组临床疗效及并发症发生率;比较2组治疗前后胰岛素样生长因子-1 (IGF-1)、血管内皮生长因子(VEGF)、骨形态发生蛋白-2 (BMP-2)水平及Harris评分。结果:研究组总有效率96.0%,高于对照组87.5%(P<0.05)。治疗后,2组VEGF、BMP-2、IGF-1水平及Harris评分均较治疗前升高(P<0.05),研究组VEGF、BMP-2、IGF-1水平及Harris评分均高于对照组(P<0.05)。研究组并发症发生率10.0%,低于对照组21.4%(P<0.05)。结论:空心钉内固定术后给予三七接骨丸治疗,可有效改善老年股骨颈骨折患者的髋关节功能,促进骨愈合,降低术后并发症发生率。Objective:To observe the clinical effect of Sanqi Jiegu pills combined with internal fixation with hollow screw for femoral neck fracture in the senile.Methods:A total of 112 cases of senile patients with femoral neck fracture were selected and divided into the study group and the control group according to the random number table method,with 56 cases in each group respectively,6 cases were excluded from the study group,50 cases were left.Both groups were treated with internal fixation with hollow screw.After surgery,the control group was routinely treated with analgesia,anti-infection and prevention of osteoporosis,and the study group was additionally treated with Sanqi Jiegu pills based on the treatment of the control group.The clinical effects and the incidence of complications in the two groups were compared.Before and after treatment,the levels of insulin-like growth factor-1(IGF-1),vascular endothelial growth factor(VEGF) and bone morphogenetic protein-2(BMP-2) as well as the Harris scores in the two groups were compared.Results:The total effective rate was 96.0% in the study group,higher than that of 87.5% in the control group(P<0.05).After treatment,the levels of VEGF,BMP-2 and IGF-1as well as the Harris scores in the two groups were increased when compared with those before treatment(P<0.05),and the levels of VEGF,BMP-2 and IGF-1 as well as the Harris score in the study group were higher than those in the control group(P<0.05).The total incidence of complications was 10.0% in the study group,lower than that of 21.4% in the control group(P<0.05).Conclusion:After internal fixation with hollow screw,the treatment of Sanqi Jiegu pills for senile patients with femoral neck fracture can effectively improve their hip joint function and bone healing,and reduce the incidence of complications after surgery.
