摘  要:目的探讨内皮素(ET)、一氧化氮(NO)水平与糖尿病视神经病变(DON)的相关性。方法选取2020年6~9月于邯郸市第一医院眼科收治的T2DM合并DON患者(DON组)60例,同期医院内分泌科收治的单纯T2DM患者(T2DM组)60例,另选取同期健康体检者60名为正常对照(NC)组,比较各组一般资料及生化指标,Logistic回归分析DON的影响因素,受试者工作特征(ROC)曲线评价ET、一氧化氮(NO)水平对DON的预测价值。结果与NC组比较,DON、T2DM组TG、TC、HbA1c、VEGF、AngⅡ、ET水平升高,NO水平降低(P<0.05);与T2DM组比较,DON组TG、TC、HbA1c、VEGF、AngⅡ、ET水平升高,NO水平降低(P<0.05)。Logistic回归分析显示,VEGF、AngⅡ、ET、NO是DON的影响因素。血浆ET、NO联合检验的ROC曲线下面积为0.812,敏感度与特异度分别为72.16%、87.23%。结论DON患者血浆ET、NO水平表达异常,对DON有良好预测价值。Objective To investigate the correlation between endothelin(ET)and nitric oxide(NO)expression levels and diabetic optic neuropathy(DON).Methods A total of 180 subjects were enrolled in this study and divided into three groups:patients with T2DM and DON admitted to The Ophthalmology Department of Handan First Hospital from June 2020 to September 2020(DON group,n=60),patients with T2DM alone admitted to the endocrinology department of the hospital during the same period(T2DM group,n=60),and another 60 healthy physical examiners during the same period(NC group,n=60).The general data and biochemical indexes were compared among the three groups.The influencing factors for DON were analyzed by logistic regression,and the predictive value of ET and NO levels on DON was analyzed by receiver operator characteristic(ROC)curves.Results TG,TC,HbA1c,VEGF,Ang II,ET levels were higher and NO levels were lower in the DON and T2DM groups compared with NC group(P<0.05).Compared with the T2DM group,the DON group had higher levels of TG,TC,HbA1c,VEGF,AngⅡ,and ET and lower levels of NO(P<0.05).Logistic regression analysis showed that VEGF,AngⅡ,ET,and NO were all influencing factors for DON.The AUC of the plasma ET and NO combined test was 0.812,and the sensitivity and specificity were 72.16%and 87.23%,respectively.Conclusion The abnormal expression of plasma ET and NO levels has good predictive value for DON.
