摘  要:目的探讨运动行为对老年T2DM患者血清脑源性神经营养因子(BDNF)、甲基乙二醛(MG)及认知功能的影响。方法选取2014年2月至2015年3月于兰州大学第二医院内分泌代谢科住院的老年T2DM患者(T2DM组)64例,另选取同期本院体检的健康老年人群57名为正常对照(NC)组。制定运动方案及随访计划并进行运动指导。比较两组运动前、运动干预12及24周的BMI、TC、LDL-C、TG、HDL-C、HbA_(1)c、FPG、2 hPG、血清BDNF、MG和认知功能及重复性成套心理状态测验分数。结果T2DM组运动12周与运动前相比,BMI、TC、TG降低,即刻记忆、延时记忆改善(P<0.05),但对视觉广度、言语功能、TG影响差异无统计学意义(P>0.05);运动对血清BDNF水平有一定提高,但对血清MG变化差异无统计学意义(P>0.05)。T2DM组运动24周与运动前相比,BMI、TC、TG、HbA_(1)c持续降低(P<0.05),LDL-C开始降低(P<0.05);认知评估中即刻记忆、延时记忆及言语功能得分升高(P<0.05),视觉广度无明显改变(P>0.05);运动24周后血清BDNF升高,MG降低(P<0.05)。运动24周后T2DM组言语功能、延时记忆得分低于NC组(P<0.05)。Pearson相关分析显示,运动干预时间与延迟记忆、言语功能、标化总分、血清BDNF呈正相关(P<0.05),与BMI、HbA^(1)c、FPG、2 h PG、TC、LDL-C、TG及MG呈负相关(P<0.05)。结论适当运动对老年T2DM患者外周血BDNF产生积极影响,同时减少外周血清MG浓度,对改善认知障碍起重要作用。Objective To investigate the effect of exercise behavior on serum brain-derived neurotrophic factor(BDNF),methylglycolic(MG)and cognitive function in elderly patients with type 2 diabetes mellitus(T2DM).Methods 64 elderly T2DM patients(T2DM group)hospitalized in the Second Hospital of Lanzhou University from February 2014 to March 2015 were recruited.57 healthy elderly people who underwent physical examination in the same period were selected as the normal control(NC)group.Exercise programs and follow-up plans were customized for all subjects.Body mass index(BMI),TC,LDL-C,TG,HDL-C,glycosylated hemoglobin(HbA_(1)c),fasting blood glucose(FPG),2 hours postprandial blood glucose(2 hPG),serum BDNF,MG and cognitive function were compared between the two groups at baseline and at 12 and 24 weeks of exercise intervention.Cognitive function was evaluated according to the reproducibility test of mental states.Results In T2DM group,serum TC,TG and BMI were decreased after 12weeks of exercise,and immediate memory(P<0.01)and delayed memory(P<0.01)were significantly improved,but visual span,speech function and TG were not significantly affected(P>0.05).Exercise increased the serum BDNF level(P<0.01),but not the serum MG level(P>0.05).Compared with before exercise,BMI(P<0.01),TC(P<0.05)and TG(P<0.01)decreased in diabetic group after 24 weeks of exercise,and LDL-C(P<0.05)decreased significantly.In the cognitive assessment,the scores of immediate memory(P<0.01)and delayed memory(P<0.01)continued to increase,and the scores of speech function(P<0.05)also increased,while the visual span(P<0.05)had no significant effect.Twenty-four weeks after the start of the experiment,peripheral serum BDNF concentration continued to increase(P<0.01),and serum MG level decreased(P<0.05).After 24 weeks of exercise,the scores of speech function and delayed memory in T2DM group were lower than those in NC group(P<0.05).Pearson correlation analysis showed that the time of exercise intervention was positively correlated with delayed memory,speech functi
