痔上黏膜柱状缝扎加改良皮桥切除缝合内痔结扎术治疗重度脱垂痔22例疗效观察

摘　　要：目的:观察痔上黏膜柱状缝扎加改良皮桥切除缝合内痔结扎术治疗重度脱垂痔的疗效。方法:将50例重度脱垂痔患者分为观察组22例和对照组28例。观察组患者采用痔上黏膜柱状缝扎加改良皮桥切除缝合内痔结扎术治疗,对照组患者采用外剥内扎术治疗,比较2组患者的疗效、术后出血及疼痛评分。结果:2组患者治疗总有效率比较差异无统计学意义(P>0.05)。观察组患者术后Ⅰ级出血13例(59.09%),Ⅱ级出血9例(40.91%);对照组患者术后Ⅰ级出血7例(25.00%),Ⅱ级出血14例(50.00%),Ⅲ级出血7例(25.00%);2组比较差异有统计学意义(P<0.05)。观察组患者治疗3、7、14d的疼痛评分均低于对照组(P<0.05)。结论:痔上黏膜柱状缝扎加改良皮桥切除缝合内痔结扎术治疗重度脱垂痔具有出血少、疼痛轻、恢复快等优点。Objective To observe the effect of columnar ligature&suture for prolapse and hemorrhoids(CLSPH)plus modified resection&suture for skin tag and ligature of inner hemorrhoids(modified RSST and LIH).Methods Enrolled and divided 50patients with severe prolapsed hemorrhoids treated in author’s hospital(2018-02-2021-06)into observation group(22cases,received above CLSPH plus modified RSST and LIH)and control group(28cases,external dissection&internal ligature);then,compared both groups’effect,postoperative bleeding status,and the score ratings on pain.Results In postoperative bleeding status there was statistical difference between observation group and control group[Ⅰgrade:in13cases(59.09%)vs in 7(25.00%),Ⅱgrade:in 9cases(40.91%)vs in 14(50.00%),and,Ⅲgrade:0vs in 7(25.00%),thus,P<0.05];in total effective rate there was no statistical difference between the two groups(P>0.05);As for the ratings on pain at 3,7and 14days after treatment observation group was lower than control group(P<0.05).Conclusion The combined procedure used in observation group for severe prolapsed hemorrhoids has such merits as less bleeding,slighter pain,and faster recovery.
