甲氨蝶呤联合米非司酮治疗异位妊娠的临床疗效及不良反应

摘　　要：目的分析异位妊娠采取甲氨蝶呤联合米非司酮治疗的临床疗效及不良反应。方法76例异位妊娠患者,基于随机数字法分为参照组和研究组,每组38例。参照组给予甲氨蝶呤治疗,研究组给予甲氨蝶呤联合米非司酮治疗。对比两组患者临床疗效、治疗前后血清β-人绒毛膜促性腺激素(β-HCG)水平及包块直径、不良反应发生率。结果研究组治疗总有效率89.47%高于参照组的68.42%,差异具有统计学意义(P<0.05)。治疗前,两组患者血清β-HCG水平及包块直径比较,差异无统计学意义(P>0.05);治疗后,两组患者血清β-HCG水平低于本组治疗前、包块直径小于本组治疗前,且研究组患者血清β-HCG水平(1251.33±87.68)IU/L低于参照组的(1365.37±94.75)IU/L,包块直径(1.52±0.26)mm小于参照组的(2.17±0.71)mm,差异具有统计学意义(P<0.05)。研究组患者的不良反应发生率7.89%低于参照组的26.32%,差异具有统计学意义(P<0.05)。结论甲氨蝶呤联合米非司酮对于异位妊娠的临床价值显著,能够进一步提升治疗效果,降低患者不良反应,安全性较高,值得在临床中推广使用。Objective To analyze the clinical efficacy and adverse reactions of methotrexate combined with mifepristone in the treatment of ectopic pregnancy.Methods A total of 76 patients with ectopic pregnancy were divided into reference group and research group according to random numerical table,with 38 cases in each group.The control group was treated with methotrexate,and the research group was treated with methotrexate combined with mifepristone.The clinical efficacy,serumβ-human chorionic gonadotropin(β-HCG)level before and after treatment,mass diameter,and incidence of adverse reactions were compared between the two groups.Results The total effective rate 89.47%of the research group was higher than 68.42%of the reference group,and the difference was statistically significant(P<0.05).Before treatment,there was no statistically significant difference in serumβ-HCG level and mass diameter between the two groups(P>0.05).After treatment,the serumβ-HCG levels of the two groups were lower than those of this group before treatment,and the mass diameter was smaller than that of this group before treatment;the serumβ-HCG level(1251.33±87.68)IU/L of the research group were lower than(1365.37±94.75)IU/L of the reference group;the mass diameter(1.52±0.26)mm of the research group was smaller than(2.17±0.71)mm of the reference group;all the differences were statistically significant(P<0.05).The incidence of adverse reactions 7.89%in the research group was lower than 26.32%in the reference group,and the difference was statistically significant(P<0.05).Conclusion Methotrexate combined with mifepristone has great clinical value in the treatment of ectopic pregnancy,which can further enhance the therapeutic effect,reduce the adverse reactions of patients,and has high safety.It is worthy of clinical promotion and application.
