2016~2020年南通市第三人民医院核苷类抗病毒药使用分析

摘　　要：目的了解南通市第三人民医院(以下简称“该院”)核苷类抗病毒药的使用情况及用药趋势,为临床合理应用提供一定的参考。方法选取该院2016~2020年核苷类抗病毒药的相关信息,统计其使用数量、销售金额、用药频度(DDDs)、排序比(B/A)及限定日费用(DDC)等数据。结果2017年销售金额为1216.80万元,较2016年增长47.55%,2018年、2019年维持稳态水平,2020年为654.69万元,较2019年下降45.22%;恩替卡韦和替诺福韦的诸多规格在该院5年间的使用最为广泛;2016~2019年DDDs均居首位的为恩替卡韦胶囊(恩甘定),2020年则是4+7恩替卡韦排名第1;替诺福韦二吡呋酯片(韦瑞德)、替比夫定片、阿德福韦酯片(优贺丁)、阿德福韦酯片(阿甘定)B/A在0.5~1.5之间,其余药品B/A波动较大;恩替卡韦片(博路定)、替比夫定片、替诺福韦二吡呋酯片(韦瑞德)DDC位居前3位,均大于10元;4+7恩替卡韦、4+7替诺福韦二吡呋酯片、4+7阿德福韦酯片DDC较小,均低于1元。结论该院2020年核苷类抗病毒药销售金额大幅下降,为实施4+7集中带量采购药品价格下降所致,切实减轻了患者的药费负担。Objective To understand the usage and trend of nucleoside antiviral drugs in Nantong Third People's Hospital(hereinafter referred to as"the hospital"),so as to provide some reference for rational clinical application.Methods The relevant information of nucleoside antiviral drugs in our hospital from 2016 to 2020 was selected,and the data of quantity,consumption sum,DDDs,ordering ratio(B/A)and limited daily cost(DDC)of nucleoside antiviral drugs were analyzed.Results In 2017,the consumption sum was 12.168 million yuan,increasing by 47.55% compared with 2016.In 2018 and 2019,it maintained a steady level,and in 2020,it was 6.5469 million yuan,decreasing by 45.22% compared with 2019.Entecavir and Tenofovir specifications were the most widely used in the hospital during the five-year period;From 2016 to 2019,Entecavir capsule ranked first in DDDs,and in 2020,4+7 Entecavir ranked first.The B/A of tenofovir dipiroproxil tablets(Veride),tibivudine tablets,adefovir dipivoxil tablets(Uhetin)and adefovir dipivoxil tablets(Agaldine)ranged from 0.5 to 1.5,while the B/A of other drugs fluctuated greatly.DDC distribution of entecavir tablets(Brudine),tibivudine tablets and tenofovir disoproxil tablets(Veride)were the top 3,all more than 10 yuan.DDC of 4+7 entecavir,4+7 tenofovir disoproxil and 4+7 Adefovir dipivoxil tablets were smaller than 1 yuan.Conclusion In 2020,the consumption sum of nucleoside antivirals in the hospital decreased significantly,which was caused by the implementation of 4+7 centralized procurement of drugs with reduced prices,effectively reducing the burden of drug costs of patients.
