阴道用药对妊娠期B族链球菌阳性并阴道微环境异常孕妇的临床研究

摘　　要：目的探究阴道用药对妊娠期B族链球菌阳性并阴道微环境异常孕妇的临床效果。方法选取2019年12月至2021年12月太康县人民医院接诊的150例妊娠期B族链球菌阳性并阴道微环境异常孕妇作为研究对象,按照治疗方法分组,分为A组(硝呋太尔制霉素软胶丸)、B组(乳杆菌活菌胶囊)、C组(联合治疗)各50例,对比各组治疗有效率、血清雌二醇(E_(2))、卵泡刺激素(FSH)水平、症状消失时间、不良妊娠发生率情况。结果C组治疗有效率(98.00%)高于A组、B组(84.00%、80.00%),差异有统计学意义(P<0.05)。治疗前,三组E_(2)、FSH水平对比,差异无统计学意义(P>0.05),治疗后各组E_(2)指标均高于治疗前,FSH指标均低于治疗前,且C组治疗后E_(2)指标均高于A组、B组,FSH指标均低于A组、B组,差异有统计学意义(P<0.05)。C组阴道疼痛、阴道瘙痒、阴道黏膜充血、白带等症状消失时间均短于A组、B组,差异有统计学意义(P<0.05)。C组不良妊娠发生率(10.00%)低于A组、B组(28.00%、38.00%),差异有统计学意义(P<0.05)。结论将硝呋太尔制霉素软胶丸联合乳杆菌活菌胶囊阴道用药应用于妊娠期B族链球菌阳性并阴道微环境异常孕妇后,可显著提升疗效,缓解临床症状,减少不良妊娠发生率,值得应用。Objective To explore the clinical effect of vaginal medication on pregnant women with positive group B streptococcus and abnormal vaginal microenvironment.Methods From December 2019 to December 2021,150 pregnant women with positive group B streptococcus and abnormal vaginal microenvironment who were diagnosed in Taikang County People's Hospital during pregnancy were selected as the research objects.They were divided into group A(nifuratel nysfungin vaginal soft capsules),group B(lactobacillus live bacterial gel capsule),and group C(combined treatment)according to the treatment methods.The treatment efficiency,serum estradiol(E_(2)),follicle stimulating hormone(FSH)levels,time of symptom disappearance and incidence of adverse pregnancy were compared.Results The effective rate of group C(98.00%)was significantly higher than that of group A and group B(84.00%,80.00%)(P<0.05).Before treatment,there was no significant difference in E_(2) and FSH levels among the three groups(P>0.05).After treatment,E_(2) indicators in each group were higher than those before treatment,while FSH indicators were lower than those before treatment.After treatment,E_(2) indicators in group C were higher than those in group A and group B,and FSH indicators were lower than those in group A and group B,with a statistically significant difference(P<0.05).The disappearance time of vaginal pain,vaginal pruritus,vaginal mucosal congestion,leucorrhea and other symptoms in group C was shorter than that in group A and group B(P<0.05).The incidence of adverse pregnancy in group C(10.00%)was lower than that in group A and group B(28.00%,38.00%),with a statistically significant difference(P<0.05).Conclusion The vaginal application of nifuratel nysfungin vaginal soft capsules combined with lactobacillus viable capsule in pregnant women with positive group B streptococci and abnormal vaginal microenvironment can significantly improve the efficacy,alleviate clinical symptoms,and reduce the incidence of adverse pregnancy,which is worthy of application.
