PDCA循环对于可复用器械集中消毒供应的管理效果研究

摘　　要：目的探讨PDCA循环对于可复用器械集中消毒供应的管理效果。方法纳入河南科技大学第一附属医院2021年1月至2021年12月消毒供应室处理的400件复用医疗器械,按照时间先后顺序将其中1~6月接受常规管理的200件消毒供应中心复用医疗器械作为对照组,7~12月开展PDCA管理的200件复用医疗器械作为研究组。比较不同管理模式下,两组器械的清洗消毒合格率、器械不良事件发生率、工作人员发生锐器伤、基本工作质量、工作效率以及科室满意度评价。结果(1)器械:研究组复用器械从回收至发放各个环节清洗消毒的合格率均高于对照组,差异有统计学意义(P<0.05)。研究组不良事件发生率及工作人员锐器伤发生率均低于对照组,差异有统计学意义(P<0.05),临床科室对研究组复用器械管理模式下的满意度远远高出对照组,差异有统计学意义(P<0.05)。(2)工作人员,实施PDCA管理模式后,工作人员的废弃物品分类、器械清洗效果、检查与包装、消毒灭菌效果等项目的工作效率评分均高于实施前,且院内感染知识掌握率、标准预防执行率以及手卫生执行率同样远高于实施前,差异有统计学意义(P<0.05)。结论采用PDCA循环法对消毒供应中心可复用器械进行管理,不仅能够提高器械的清洗、消毒质量,也有助于增强职工的工作质量及效率,获得满意的管理效果。Objective To explore the management effect of PDCA cycle on centralized disinfection and supply of reusable instruments.Methods A total of 400 reused medical devices were included in the disinfection and supply room of the First Affiliated Hospital of Henan University of Science and Technology from January 2021 to December 2021.Among them,200 reused medical devices received routine management in the disinfection and supply center from January to June were taken as the control group,and 200 reused medical devices under PDCA management from July to December were taken as the research group.Under different management modes,the qualification rate of cleaning and disinfection,the incidence rate of adverse events,sharp instrument injuries,basic work quality,work efficiency and department satisfaction evaluation of the two groups of instruments were compared.Results(1)Instruments:The eligible rate of cleaning and disinfection of reused instruments in the study group was higher than that in the control group from recovery to distribution,and the difference was statistically significant(P<0.05).The incidence of adverse events and sharp instrument injuries of staff in the study group were lower than those in the control group,with a statistically significant difference(P<0.05).(2)For staff,after the implementation of PDCA management mode,the work efficiency scores of the staff's items such as waste article classification,instrument cleaning effect,inspection and packaging,disinfection and sterilization effect were higher than those before the implementation,and the mastery rate of hospital infection knowledge,the implementation rate of standard prevention and the implementation rate of hand hygiene were also much higher than those before the implementation,with a statistically significant difference(P<0.05).Conclusion PDCA circulation method can not only improve the cleaning and disinfection quality of the reusable instruments in the disinfection supply center,but also help to enhance the work quality and efficiency of the
