摘  要:目的 观察穴位埋线对溃疡性结肠炎(UC)大鼠肠黏膜上皮屏障的影响,基于p38MAPK/NF-κB信号通路探讨其治疗UC的作用机制。方法 将46只SPF级雄性SD大鼠随机分为空白组10只和造模组36只,采用腺嘌呤、番泻叶分阶段灌胃,再予2,4,6-三硝基苯磺酸与乙醇复合物灌肠制备UC大鼠模型。将成模大鼠随机分为模型组、柳氮磺吡啶(SASP)组和埋线组,埋线组于“足三里”“天枢”“肾俞”“脾俞”“大肠俞”“膈俞”埋线,14 d埋线1次,共3次;SASP组予SASP混悬液灌胃,空白组和模型组予等量生理盐水灌胃,连续42 d。观察大鼠一般情况,进行结肠黏膜损伤指数(CMDI)评分,HE染色观察结肠组织病理变化,ELISA检测血清白细胞介素(IL)-1β、IL-6及肿瘤坏死因子-α(TNF-α)含量,PCR检测结肠组织p38丝裂原活化蛋白激酶(p38MAPK)、核因子(NF)-κB p65、闭锁小带蛋白-1(ZO-1)、咬合蛋白(Occludin)mRNA表达,Western blot检测结肠组织p38MAPK、NF-κB p65、Occludin、ZO-1蛋白表达。结果 与空白组比较,模型组大鼠结肠长度明显减少,体质量明显减轻,CMDI评分明显升高(P<0.01),血清IL-1β、IL-6及TNF-α含量明显增加(P<0.01),结肠组织p38MAPK、NF-κB p65 mRNA和蛋白表达明显升高(P<0.01),Occludin、ZO-1 mRNA和蛋白表达明显降低(P<0.01);与模型组比较,SASP组和埋线组大鼠结肠长度、体质量明显增加(P<0.01),CMDI评分明显降低(P<0.01),血清IL-1β、IL-6及TNF-α含量明显减少(P<0.01),结肠组织p38MAPK、NF-κB p65 mRNA和蛋白表达明显降低(P<0.01),Occludin、ZO-1 mRNA和蛋白表达明显升高(P<0.01,P<0.05)。结论 穴位埋线可增加UC大鼠紧密连接蛋白Occludin、ZO-1表达,修复肠道黏膜屏障功能,其作用机制可能与抑制p38MAPK/NF-κB通路激活有关。Objective To observe the effects of acupoint catgut embedding on the epithelial barrier function of the intestinal mucosa in rats with ulcerative colitis(UC);To explore its mechanism of action in treating UC through the p38MAPK/NF-κB signaling pathway. Methods Totally 46 SPF SD male rats were randomly divided into blank group(10 rats) and modeling group(36 rats). The UC rat model was established by intragastric administration of adenine and Folium Sennae in stages combined with 2, 4, 6-trinitrobenzene sulfonic acid and ethanol compound enema. The modeling rats were randomly divided into model group, SASP group and embedding group, with 12 rats in each group. The embedding group was treated with acupoint embedding at “Zusanli”, “Tianshu”, “Shenshu”,“Pishu”, “Dachangshu” and “Geshu”, 14 days/time, a total of 3 times;the SASP group was given SASP solution by gavage, and the blank group and model group were given same amount of normal saline by gavage for 42 days. The general condition of the rats was observed, and the colonic mucosal damage index(CMDI) was scored. HE staining was used to observe the pathological changes of colon tissue. The contents of IL-1β, IL-6 and TNF-α in serum were detected by ELISA. The expressions of p38MAPK, NF-κB p65, Occludin and ZO-1 mRNA in colon tissue were detected by PCR. The expressions of p38MAPK, NF-κB p65, Occludin and ZO-1 protein in colon tissue were detected by Western blot. Results Compared with the blank group, the colon length of rats in the model group significantly shortened, the body weight significantly reduced, the score of CMDI significantly increased(P<0.01),the contents of IL-1β, IL-6 and TNF-α in serum significantly increased(P<0.01), the mRNAs and proteins expression of p38MAPK and NF-κB p65 in colon tissue significantly increased(P<0.01), the mRNAs and proteins expression of ZO-1 and Occludin significantly decreased(P<0.01).
