三参通脉合剂治疗冠心病PCI术后合并焦虑状态的临床研究

摘　　要：目的观察三参通脉合剂治疗冠心病经皮冠状动脉介入治疗(PCI)后合并焦虑状态的临床疗效。方法选取2020年1月—2021年12月首都医科大学附属北京中医医院心血管科门诊及住院的冠心病PCI术后合并轻中度焦虑状态病人66例,按照随机数字表法分为治疗组与对照组,每组33例。两组均接受常规西药治疗,治疗组在常规西药治疗基础上加服三参通脉合剂,治疗周期为30 d。观察两组治疗前后西雅图心绞痛量表(SAQ)、中医症状及中医证候(气虚血瘀证)积分、汉密尔顿焦虑量表积分、血小板功能检测及炎性因子水平变化。结果治疗组治疗后SAQ各项积分均高于对照组(P<0.05);治疗组治疗后中医症状及气虚血瘀证积分均较对照组明显改善(P<0.05)。治疗组焦虑疗效总有效率高于对照组(90.9%与21.2%,P<0.01)。治疗组治疗后P2Y12反应单位(PRU)、血小板膜P-选择素(CD62p)含量及超敏C反应蛋白(hs-CRP)、白细胞介素6(IL-6)均低于对照组(P<0.05)。结论三参通脉合剂可有效改善冠心病PCI术后合并焦虑状态病人的SAQ积分及焦虑状态,降低中医症状及中医证候积分,改善血小板功能及降低炎性因子水平。Objective To observe the clinical efficacy of Sanshen Tongmai Oral Liquid on coronary heart disease complicated with anxiety after percutaneous coronary intervention(PCI).Methods Sixty-six patients with coronary heart disease complicated with mild to moderate anxiety after PCI were divided into treatment group(n=33)and control group(n=33)according to random number table method.The patients in both groups received conventional western medicine treatment.The patients in treatment group took Sanshen Tongmai Oral Liquid extrally,and the treatment period was 30 days.The changes of Seattle Angina Pectoris Scale(SAQ),scores of traditional Chinese medicine(TCM)symptoms and syndromes,anxiety scale,platelet function test,and inflammatory cytokine levels in both groups were observed before and after treatment.Results After treatment,compared with control group,the SAQ scores in treatment group were more than those in control group(P<0.05).After treatment,the scores of TCM symptoms,Qi deficiency and blood stasis syndrome in treatment group were significantly improved compared with those in control group(P<0.05).The total effective rate of anxiety in treatment group was significantly higher than that in control group(90.9%and 21.1%,P<0.01).The plasma platelet P2Y12 response unit(PRU),platelet membrane P-selectin(CD62p)content,hypersensitive C-reactive protein(hs-CRP),and interleukin 6(IL-6)in treatment group were less than those in control group(P<0.05).Conclusion Sanshen Tongmai Oral Liquid can effectively improve SAQ score and anxiety state of patients with coronary heart disease complicated with anxiety state after PCI,reduce TCM symptoms and TCM syndrome score,improve platelet function,and reduce the level of inflammatory factors.
