养阴解毒汤对喉癌术后放疗患者癌因性疲乏及生活质量影响

摘　　要：目的观察养阴解毒汤对喉癌术后放疗患者的癌因性疲乏及生活质量的影响。方法选取2018年4月—2021年4月收治的62例喉癌患者,按随机数字表法分为观察组与对照组,每组均31例。对照组患者仅予三维调强放疗(IMRT),放疗时间为6周。观察组于对照组的IMRT基础上予养阴解毒汤治疗,连续治疗6周。比较两组喉颈部的迟发性淋巴水肿、癌症疲乏量表(CFS)和生活质量评分。结果放疗中观察组的喉部淋巴水肿发生率显著少于对照组(P<0.05);放疗中观察组的颈部淋巴水肿发生率少于对照组,但差异无统计学意义(P>0.05);治疗后,对照组患者的癌症疲乏量表(CFS)指标躯体疲乏、情感疲乏、认知疲乏评分较治疗前显著增加(P<0.01),但观察组治疗前后以上指标评分差异无统计学意义(P>0.05),且观察组治疗后躯体疲乏、情感疲乏、认知疲乏评分明显低于对照组(P<0.01);治疗后,观察组癌症患者生活质量核心调查问卷(EORTC QLQ-C30)指标躯体、角色、情绪、认知、社会功能评分以及疲劳、疼痛、恶心呕吐评分改善都显著优于对照组(P<0.01)。结论对喉癌术后放疗患者给予养阴解毒汤内服干预可有效减轻喉颈部淋巴水肿,改善患者的癌因性疲乏及生活质量。Objective To observe the efficacy of Yangyin Jiedu Decoction(养阴解毒汤)on cancerrelated fatigue and life quality of postoperative laryngeal cancer patients with radiotherapy.Methods Sixtytwo laryngeal cancer patients admitted to our hospital from April of 2018 to April of 2021 were selected and divided into observation group and control group according to random number table method with 31cases in each group.The control group only received three-dimensional intensity modulated radiotherapy(IMRT)for 6 weeks.The observation group was treated with Yangyin Jiedu Decoction on the basis of IMRT of control group for 6 weeks.Delayed lymphedema,scores of cancer fatigue scale(CFS)and life quality were compared between the two groups.Results The incidence of laryngeal lymphedema in the observation group was significantly lower than that in the control group(P<0.05).During radiotherapy,the incidence of neck lymphedema in the observation group was less than that in the control group,but the difference was not statistically significant(P>0.05).After treatment,scores of physical fatigue,emotional fatigue and cognitive fatigue of cancer fatigue scale(CFS)in control group were significantly increased compared with those before treatment(P<0.01),but there was no statistical significance in the scores of CFS in observation group before and after treatment(P>0.05).The scores of physical fatigue,emotional fatigue and cognitive fatigue in observation group were significantly lower than those in control group(P<0.01).After treatment,the body,role,emotion,cognition,social function scores,fatigue,pain,nausea and vomiting scores in the observation group were significantly better than those in the control group(P<0.01).Conclusion The intervention of Yangyin Jiedu Decoction on postoperative laryngeal cancer patients with radiotherapy can effectively reduce laryngeal and neck lymphedema and improve the cancer fatigue and life quality of patients.
