摘 要:目的探讨应用TiRobot手术机器人辅助经皮无头加压螺钉内固定治疗新鲜腕舟骨腰部骨折的手术方法及临床效果。方法回顾性分析2017年1月-2020年1月应用TiRobot手术机器人辅助经皮无头加压螺钉内固定治疗新鲜腕舟骨腰部骨折10例的病例资料,总结切口长度、术中导针调整次数、螺钉位置及长度、手术时间、X线透视时长等临床指标。术后随访12~18个月,评价骨折愈合、腕关节功能恢复等临床效果。结果本组病例的切口长度(6.30±1.16)mm,术中主导针调整次数(1.50±0.53)次,螺钉均位于舟骨中央位置,螺钉长度(24.10±1.37)mm,螺钉与轴线角度为(7.10±4.09)°,手术时间为(58.80±10.32)min,X线曝光时间为(17.24±1.50)s,骨折愈合时间为(10.40±2.80)周。术后12周腕关节活动度均达到正常范围,患者无明显不适感,Mayo评分(93.50±4.74)分。末次随访时未见创伤性关节炎、腕关节僵硬等相关并发症。结论应用TiRobot手术机器人辅助经皮无头加压螺钉内固定治疗新鲜腕舟骨腰部骨折是一种安全、微创的手术方法,术后治愈率高、并发症少。Objective To investigate the surgical method and clinical effect of TiRobot surgical robot-assisted percutaneous compression screw internal fixation in the treatment of fresh scaphoid fracture.Methods The clinical data of 10 patients with fresh scaphoid fracture treated with robot-assisted percutaneous compression screw internal fixation in our hospital from January 2017 to January 2020 were retrospectively analyzed.The length of the incision,the times of adjusting the guide needle during the operation,the position and length of the screw,the operation time and the X-ray fluoroscopy time were analyzed.The clinical effect of fracture healing and functional recovery of wrist joint was evaluated after 12-18 months of follow-up.Results In this group of patients,the incision length was(6.30±1.16)mm,the number of guide adjustments was(1.50±0.53)times,the screws were all located in the central position of the scaphoid,the screw length was(24.10±1.37)mm,and the operation time was(58.80±10.32)min.The X-ray exposure time was(17.24±1.50)s.The range of motion of the wrist joint reached the normal range at 12 weeks after operation,and the patient had no obvious discomfort,and the Mayo score was(93.50±4.74).No traumatic arthritis,wrist stiffness and other related complications were found at the last follow-up.Conclusion The application of TiRobot orthopedic surgery robot assisted percutaneous compression screw internal fixation for the treatment of fresh scaphoid lumbar fracture is a safe and minimally invasive surgical method with high postoperative healing rate and few complications.
