多学科综合治疗糖尿病足76例

摘　　要：目的总结糖尿病足多学科治疗的经验。方法2018年1月-2021年1月共收治糖尿病足患者76例。病程1~12个月,平均46个月。Wagner分级:2级19例,3级37例,4级20例。根据患者病情采用降糖、抗炎、清创、负压吸引、截肢、胫骨横向骨搬运、换药、植皮或皮瓣移植、疏通下肢血管等多学科综合治疗。术后统计创面愈合时间,评价治疗效果。结果所有患者均获随访1~16个月,平均7.8个月;溃疡创面均愈合,愈合时间3.1~9.2周,平均5.2周。结论糖尿病足的治疗既要关注局部,也要关注全身,早期评估患者病情,采用多学科综合治疗,可提高治疗效果,减少并发症发生。Objective To summarize the experience of multidisciplinary treatment of diabetic foot.Methods From January 2018 to January 2021,76 diabetic foot patients were admitted to our department.The course of disease ranged from 1 to 12 months,with an average of 1.6 months.Wagner grade:19 cases of grade 2,37 cases of grade 3 and 20 cases of grade 4.According to the patient's condition,the multidisciplinary technology of hypoglycemic,anti-inflammatory,debridement,negative pressure suction,amputation,transverse tibial bone transportation,dressing change,skin grafting or skin flap transplantation and dredging the blood vessels of lower limbs were adopted.The wound healing time was counted and the treatment effect was evaluated.Results The patients were followed up for 1 to 16 months,with an average of 7.8 months;The ulcer wounds healed,and the healing time was 3.1 to 9.2 weeks,with an average of 5.2 weeks.Conclusion The treatment of diabetic foot should pay attention to both the local and the whole body.The main contradiction of the patient's condition should be evaluated at the early stage.Multidisciplinary treatment can improve the therapeutic effect and reduce the incidence of complications.
