手指末节伸肌腱止点撕脱性骨折的微创治疗

摘　　要：目的探讨手指末节指伸肌腱止点撕脱性骨折微创治疗的临床效果。方法2019年1月-2021年3月,应用克氏针微创纵行固定远侧指骨间关节对15例手指伸肌腱止点撕脱性骨折进行手术治疗,术后6~8周拔除克氏针,进行主动手指关节屈伸功能训练。结果术后15例全部得到随访,随访时间6~24个月。15例撕脱骨块均骨性愈合,远侧指骨间关节间隙正常,关节面平整,无下垂畸形,关节无疼痛。采用Dargan功能评定法评定疗效,优12例,良3例,优良率100%。结论应用克氏针微创纵行固定远侧指骨间关节治疗手指末节指伸肌腱撕脱性骨折,不用切开复位骨块,手术微创,骨块愈合好,关节面平整,远侧指骨间关节功能恢复良好,值得基层医院推广。Objective To explore the clinical effect of minimally invasive treatment of avulsion fracture of extensor tendon of finger end.Methods From January 2019 to June 2021,15 cases of avulsion fracture of extensor tendon insertion point of fingers were treated by minimally invasive longitudinal fixation of distal interphalangeal joint with Kirschner wire.The Kirschner wire was removed 6 to 8 weeks after operation,and the flexion and extension function exercise of distal interphalangeal joint was performed.Results All 15 cases were followed up for 6 months to 2 years.All the avulsed bones healed,the distal interphalangeal joint space was normal,the joint surface was smooth,there was no sagging deformity,and the joint was pain free.Dargan function evaluation method was used to evaluate the curative effect.12 cases were excellent,3 cases were good,and the excellent and good rate was 100%.Conclusion The application of Kirschner wire minimally invasive longitudinal fixation of distal interphalangeal joint of fingers in the treatment of avulsion fracture of extensor tendon of fingers at the end of fingers,without open reduction of bone block,minimally invasive operation,good healing of bone block,smooth joint surface,good recovery of function of distal interphalangeal joint of fingers,is worthy of promotion in the basic hospitals.
