摘　　要：目的探讨旋股外侧动脉降支穿支逆行岛状皮瓣修复膝周围软组织缺损的临床疗效。方法2018年1月-2021年1月收治10例膝周围复合软组织损伤患者,皮肤缺损面积:5.0 cm×8.0 cm~11.0 cm×16.0 cm,待创面稳定后行旋股外侧动脉降支穿支逆行岛状皮瓣修复术。结果所有患者均获得随访,随访时间6~24个月,平均13.5月。末次随访时皮瓣外形饱满,皮瓣感觉S2~S3级,两点辨别觉为7.0~11.0 mm,平均9.5 mm。供区均一期愈合,呈线性瘢痕。患肢膝关节屈曲活动度为100°~130°,平均112°。伸直为-5°~0°,平均-1°。膝关节功能评定:优6例,良4例,均能满足日常生活所需。结论旋股外侧动脉降支穿支逆行岛状皮瓣修复膝周围软组织缺损的临床效果良好,可作为临床治疗膝周围软组织缺损的参考方案。Objective To investigate the clinical effect of retrograde island perforator flap pedicled with the descending branch of lateral circumflex femoral artery in repairing soft tissue defect around knee.Methods A retrospective analysis was conducted on 10 patients with complex soft tissue injury around the knee,from January 2018 to January 2021.After the wound surface was clean,it was repaired with retrograde island perforator flap pedicled with the descending branch of lateral circumflex femoral artery.Results All patients got a 6~24 months'follow-up visiting,with a mean of 13.5 months.At the last follow-up,the flap sensation was S2~S3,and the two-point discrimination was 7.0~11.0 cm,with an average of 9.5 cm.The doner area of flaps properly healed with a linear scar.The range of motion of knee flexion of the affected limb ranged from 100°~130°,with an average of 112°.The elongation was-5°~0°,with an average of-1°.Through the functional assessment,with 6 excellent cases and 4 good cases,the knee joint function could meet patients'daily demand.Conclusion The retrograde island perforator flap pedicled with the descending branch of lateral circumflex femoral artery has a good clinical effect in repairing soft tissue defects around the knee,which can be used as a reference solution for the treatment of soft tissue defects around the knee in the future.
