摘  要:目的:探讨不同脐带处理方式对新生儿围产期结局及婴儿早期铁储备的影响。方法:收集我院出生的胎龄37~42周足月儿及产妇作为研究对象,按照不同脐带处理方式分为脐带挤压组(UCM组)、延迟脐带结扎组(DCC组),并收集立即脐带结扎组(ICC组)作为对照,分别检测各组新生儿生后第3天、生后6周、生后4个月的血红蛋白(Hb)、红细胞压积(HCT)、血清铁蛋白(SF)水平,并统计新生儿高胆红素血症及新生儿红细胞增多症的发生率、婴儿期缺铁性贫血的发生率以及孕产妇产后出血的发生率。结果:生后第3天,UCM组和DCC组Hb、HCT水平均高于ICC组(P<0.05),其中UCM组与DCC组Hb、HCT水平差异无统计学意义(P>0.05);生后第6周及生后4个月,UCM组、DCC组、ICC组Hb、HCT水平相比较,差异无统计学意义(P>0.05);生后第3天,三组SF水平相比,差异无统计学意义(P>0.05);生后第6周及生后4个月,UCM组和DCC组SF水平均高于ICC组(P<0.05),其中UCM组和DCC组SF水平比较无统计学差异(P>0.05);三组新生儿高胆红素血症及新生儿红细胞增多症的发生率相比较,差异无统计学意义(P>0.05);UCM组和DCC组婴儿缺铁性贫血发生率低于ICC组(P<0.05),其中UCM组和DCC组缺铁性贫血相比较,差异无统计学意义(P>0.05)。结论:UCM和DCC对于提高新生儿血容量、增加婴儿早期铁储备及减少缺铁性贫血的发生率均有显著效果且作用相似,均不增加新生儿高胆红素血症及新生儿红细胞增多症的发生率。Objective:To explore the effects of different umbilical cord clamping methods on perinatal outcomes and early iron reserves of infants.Methods:Term infants with gestational age of 37~42 weeks and their mothers were collected as the research objects, according to different umbilical cord clamping methods, they were divided into Umbilical Cord Milking group(UCM group) and Delayed Cord Clamping group(DCC group),the Immediate Cord Clamping group(ICC group) was collected as the control, the levels of hemoglobin(Hb), hematocrit(HCT) and serum ferritin(SF) were detected on the 3 rd day, 6 th week and 4 th month after birth, the incidence rate of incidence of neonatal hyperbilirubinemia and neonatal polycythemia, incidence rate of iron deficiency anemia in infancy and incidence rate of postpartum hemorrhage were statistically analyzed.Results:At the 3 rd day after birth, the levels of Hb and HCT in UCM group and DCC group were higher than those in ICC group(P<0.05),there was no significant difference in Hb and HCT levels between UCM group and DCC group(P>0.05).There was no significant difference in Hb and HCT levels among UCM group, DCC group and ICC group at 6 weeks and 4 months after birth(P>0.05).At the 3 rd day after birth, there was no significant difference in SF levels among UCM group, DCC group and ICC group(P>0.05). At 6 weeks and 4 months after birth, SF levels in UCM group and DCC group were higher than those in ICC group(P<0.05),there was no significant difference in SF level between UCM group and DCC group(P>0.05).There was no significant difference in the incidence rate of neonatal hyperbilirubinemia and neonatal polycythemia among the three groups(P>0.05).The incidence rate of iron deficiency anemia in UCM group and DCC group was lower than that in ICC group(P<0.05).There was no significant difference in the incidence rate of iron deficiency anemia between UCM group and DCC group(P>0.05).Conclusion:UCM and DCC have similar effects on improving neonatal blood volume, increasing iron reserve and reducing inc
