摘 要:目的:探讨万州地区新型冠状病毒肺炎患者的中医临床证候特征和用药规律,为新型冠状病毒肺炎中医临床辨治提供参考。方法:收集2020年1月-2020年3月万州地区确诊新冠肺炎的患者,记录患者症状特点,采集用药处方,记录胸部CT、白细胞、淋巴细胞、CRP变化,分析并总结新冠肺炎患者中医证候特征和用药规律。结果:收集万州地区196例新型冠状病毒肺炎患者,患者症状发生频率前5位的是:纳差(82.14%),咳嗽(65.31%),乏力(17.86%),恶寒(16.84%)(发热17.35%)。处方频率前5位的是:沙参麦冬汤(38.22%),四君子汤(37.7%),麻杏石甘汤(31.94%),三仁汤(15.71%),小柴胡汤(12.04%)。中医证候评分出院前较入院时下降(P<0.05),CT有效率为72.4%,出院前与入院时中医症候评分、白细胞、淋巴细胞、CRP前后比较差异有统计学意义(P<0.05)。结论:万州地区新型冠状病毒肺炎患者临床症状以纳差、咳嗽、乏力为主,处方用药多以沙参麦冬汤、四君子汤、麻杏石甘汤、三仁汤、小柴胡汤加减为主。疾病发展过程为外感初期邪热壅肺,中期邪在少阳,后期耗气伤阴,湿邪热化为万州地区新冠肺炎患者重要特征。Objective:To explore the TCM clinical syndrome characteristics and medication rules of COVID-19patients in Wanzhou,and provide references for the clinical differentiation and treatment of COVID-19in TCM.Methods:Patients diagnosed with COVID-19in Wanzhou district were collected,their symptoms and characteristics were recorded,drug prescriptions were collected,chest CT,white blood cells,lymphocytes,and CRP were recorded,and TCM syndrome characteristics and medication rules of COVID-19patients were analyzed and summarized.Results:A total of 196coVID-19patients were collected,and the top 5frequency of symptoms were:poor appetite(82.14%),cough(65.31%),fatigue(17.86%),aversion to cold(16.84%),and fever(17.35%).The top 5prescription frequencies are:Sashen Maidong Soup(38.22%),Sijunzi Soup(37.7%),Maxing Shigan Soup(31.94%),Sanren Soup(15.71%)and Xiaochaihu soup(12.04%).The TCM syndrome score before discharge was lower than that on admission(P<0.05),and the CT effective rate was 72.4%.There were statistically significant differences in TCM syndrome score,white blood cells,lymphocytes,and CRP before and after discharge from hospital compared with that before admission(P<0.05).Conclusion:The clinical symptoms of COVID-19patients in Wanzhou district were mainly poor appetite,cough and fatigue,and the prescription drugs were mostly Sashen Maidong Soup,Sijunzi soup,Maxing Shigan soup,Sanren soup and Xiaochaihu soup.The development process of the disease is as follows:initial exogenous pathogenic heat obstructing the lung,intermediate pathogenic heat in shaoyang,later Deficiency of both Qi and Yin,and humid pathogenic heat heating,which are important characteristics of COVID-19patients in Wanzhou.
