穴位埋线治疗多囊卵巢综合征临床研究

摘　　要：目的:探究以健脾祛痰理论为指导的穴位埋线对多囊卵巢综合征(PCOS)患者的疗效及血清网膜素-1(Omentin-1)水平的影响。方法:将纳入的108例多囊卵巢综合征患者随机分为对照组和观察组各54例。对照组采用盐酸二甲双胍、来曲唑片治疗,观察组在对照组基础上加用穴位埋线治疗。观察两组患者临床疗效,比较血清性激素水平、胰岛功能相关指标、排卵周期数及血清Omentin-1水平的变化情况,统计6个月内累积妊娠率及治疗相关不良反应。结果:(1)治疗后,观察组、对照组总有效率分别为90.74%、79.63%,观察组临床疗效优于对照组(P<0.05)。治疗后观察组血清总睾酮(TT)、促黄体生成素(LH)、空腹血糖(FPG)、空腹胰岛素(FINS)、胰岛素抵抗指数(HOMA-IR)明显低于对照组(P<0.05)。治疗后观察组排卵周期数、血清Omentin-1水平明显高于对照组(P<0.05)。治疗后6个月内,观察组和对照组的累积妊娠率分别为62.96%、35.19%,观察组累积妊娠率明显高于对照组(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:以健脾祛痰理论为指导的穴位埋线治疗方法可提高PCOS患者的临床疗效,改善内分泌紊乱和胰岛素抵抗程度,促进排卵及妊娠成功率的提升,其机理可能与调节血清Omentin-1水平有关。Objective:To explore the effect of acupoint catgut embedding guided by the theory of invigorating the spleen and eliminating phlegm on the efficacy and serum omentin-1level of patients with polycystic ovary syndrome(PCOS).Methods:108patients with polycystic ovary syndrome were randomly divided into control group and observation group with 54cases in each group.The control group was treated with metformin hydrochloride and letrozole tablets,and the observation group was treated with acupoint catgut embedding on the basis of the control group.The clinical effects of the two groups were observed,and the changes of serum sex hormone levels,islet function related indicators,ovulation cycles and serum omentin-1levels were compared.The cumulative pregnancy rate and treatment-related adverse reactions within 6months were counted.Results:(1)after treatment,the total effective rates of the observation group and the control group were 90.74%and 79.63%respectively.The clinical efficacy of the observation group was better than that of the control group(P<0.05).After treatment,the serum total testosterone(TT),luteinizing hormone(LH),fasting blood glucose(FPG),fasting insulin(fins)and insulin resistance index(HOMA-IR)in the observation group were significantly lower than those in the control group(P<0.05).After treatment,the number of ovulation cycles and serum omentin-1level in the observation group were significantly higher than those in the control group(P<0.05).Within 6months after treatment,the cumulative pregnancy rates of the observation group and the control group were 62.96%and 35.19%respectively.The cumulative pregnancy rate of the observation group was significantly higher than that of the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Acupoint catgut embedding therapy guided by the theory of invigorating the spleen and eliminating phlegm can improve the clinical efficacy of PCOS patients,improve the degree of endocrine disorde
