保产无忧散在足月胎膜早破阴道试产中的疗效评价

摘　　要：目的:探讨保产无忧散应用于足月胎膜早破的疗效及对胎盘Cx-43的影响。方法:选择符合纳入诊断标准的足月胎膜早破患者120例,按照随机数字表法分为试验组(保产无忧散+催产素引产)和对照组(催产素引产)各60例。检测两组分娩前后血液感染指标、产程发动时间、总产程及产时阴道出血量、产后胎盘组织中Cx-43蛋白的表达。对比两组患者临床疗效、产程中各类相关指标、围生期母儿结局。结果:试验组患者临床总有效率为91.67%,高于对照组的70.00%,差异有统计学意义(P<0.05);对照组患者分娩后血常规及PCT与本组治疗前比较,差异有统计学意义(P<0.05);试验组阴道分娩患者产程发动时间较对照组早、总产程及产时阴道出血量均明显低于对照组,差异有统计学意义(P<0.05);试验组患者胎盘Cx-43蛋白表达水平显著高于对照组,差异有统计学意义(P<0.05);试验组患者剖宫产率及绒毛膜羊膜炎发生率均明显低于对照组,差异有统计学意义(P<0.05),但两组新生儿不良事件发生率比较差异无统计学意义(P>0.05)。结论:口服保产无忧散可促进足月胎膜早破产程发动,缩短总产程,提高机体Cx-43水平,改善围生期结局。Objective:To investigate the effect of Baochan Wuyou powder on term premature rupture of membranes and its effect on placenta Cx-43.Methods:120full-term premature rupture of membranes patients who met the inclusion criteria were randomly divided into the experimental group(Baochan Wuyou powder+oxytocin induced labor)and the control group(oxytocin induced labor)with 60cases in each group.The indexes of blood infection before and after delivery,the starting time of labor process,the total labor process and the amount of vaginal bleeding during labor,and the expression of Cx-43protein in postpartum placenta were detected in the two groups.The clinical efficacy,various related indicators during labor and perinatal maternal and fetal outcomes were compared between the two groups.Results:The total clinical effective rate of the experimental group was 91.67%,higher than 70.00%of the control group,and the difference was statistically significant(P<0.05);The blood routine examination and PCT in the control group after delivery were significantly different from those before treatment(P<0.05);The starting time of labor process,total labor process and vaginal bleeding during labor in the experimental group were significantly lower than those in the control group(P<0.05);The expression level of Cx-43protein in placenta of the experimental group was significantly higher than that of the control group(P<0.05);The cesarean section rate and the incidence of chorioamnionitis in the experimental group were significantly lower than those in the control group(P<0.05),but there was no significant difference in the incidence of neonatal adverse events between the two groups(P>0.05).Conclusion:Oral Baochan Wuyou powder can promote the onset of premature labor of full-term membranes,shorten the total labor process,improve the body\s Cx-43level,and improve perinatal outcomes.
