摘  要:目的探讨荨麻疹活动度评分及miR-361表达水平与慢性自发性荨麻疹患者血液中白细胞介素-4(IL-4)、免疫球蛋白E(IgE)和组胺(HIS)等炎症因子含量的相关性。方法选择2019年8月-2020年9月荨麻疹活动度评分大于3分的慢性自发性荨麻疹患者30例为观察组;选择同期健康体检者30例为对照组。两组均采用实时荧光PCR技术测定血清中miR-361 mRNA表达水平;采用酶联免疫吸附试验测定两组血清中IL-4、IgE、干扰素-γ(IFN-γ)和HIS的含量;采用偏相关性分析观察组荨麻疹活动度评分及miR-361 mRNA表达水平与血液炎症因子含量的相关性。结果观察组miR-361 mRNA相对表达水平为9.83±2.40,对照组miR-361 mRNA相对表达水平为1.70±0.96,观察组miR-361 mRNA相对表达明显高于对照组,两组miR-361 mRNA相对表达差异有统计学意义(P<0.001)。观察组IL-4[(45.95±7.47)ng/L]、IgE[(156.70±28.49)IU/mL]、HIS[(8.75±2.46)nmol/L]水平均高于对照组[(14.25±4.39)ng/L、(38.80±7.00)IU/mL、(1.15±0.50)nmol/L](P<0.001);观察组IFN-γ[(25.86±9.26)ng/L]水平低于对照组[(66.89±7.04)ng/L](P<0.001)。偏相关性分析结果显示:观察组荨麻疹活动度评分与IgE(r=0.729)、HIS(r=0.537)含量呈正相关(P均<0.01);观察组miR-361 mRNA表达水平与IFN-γ含量呈负相关(r=-0.428,P<0.05);与IL-4(r=0.708)、IgE(r=0.628)、HIS(r=0.514)水平呈正相关(P<0.01)。结论慢性自发性荨麻疹患者血液中部分炎症因子水平与荨麻疹活动度评分存在相关性;miR-361在慢性自发性荨麻疹患者中表达升高,且与血液中炎症因子水平之间存在相关性,miR-361可能是慢性自发性荨麻疹潜在的生物标志物。Objective To investigate the correlation between urticaria activity score,miR-361 expression level and blood inflammatory factors including interleukin-4(IL-4),immunoglobulin E(IgE)and histamine(HIS)in blood of patients with chronic spontaneous urticaria(CSU).Methods Thirty patients with CSU whose urticaria activity score was more than 3 between August 2019 and September 2020 were selected as observation group,and 30 healthy physical examinees were enrolled as the control group.miR-361 mRNA expression levels were detected by real-time fluorescent PCR(RT-PCR),and the levels of IL-4,IgE,interferon-γ(IFN-γ)and HIS were detected by enzyme-linked immunosorbent assay(ELISA)in serum of both groups.The correlation of urticaria activity score and miR-361 mRNA expression level with the levels of blood inflammatory factors in observation group was analyzed by partial correlation analysis.Results The relative expression level of miR-361 mRNA was 9.83±2.40 in observation group and 1.70±0.96 in control group.The relative expression of miR-361 mRNA in the observation group was significantly higher than that in the control group,and there was a significant difference in the expression of miR-361 mRNA between the two groups(P<0.001).The levels of IL-4[(45.95±7.47)ng/L],IgE[(156.70±28.49)IU/mL]and HIS[(8.75±2.46)nmol/L]in observation group were higher than those in control group[(14.25±4.39)ng/L,(38.80±7.00)IU/mL,(1.15±0.50)nmol/L](P<0.001).The level of IFN-γin observation group[(25.86±9.26)ng/L]was lower than that in control group[(66.89±7.04)ng/L](P<0.001).Partial correlation analysis showed that urticaria activity score was positively correlated with IgE(r=0.729)and HIS(r=0.537)in the observation group(P<0.01).miR-361 mRNA expression level in observation group was negatively correlated with IFN-γlevel(r=-0.428,P<0.05),and positively correlated with IL-4(r=0.708),IgE(r=0.628),and HIS(r=0.514)(P<0.01).Conclusion The level of some inflammatory factors were correlated with urticaria activity score in patients with CSU
