摘　　要：目的了解2010-2019年广西某综合医院就诊患者HIV检测的感染状况及流行病学特征。方法收集2010年1月-2019年12月广西某综合医院门诊和住院就诊患者的HIV检测结果,利用SPSS22.0软件进行描述性统计分析。结果2010年1月-2019年12月共进行HIV筛查964243人份,HIV筛查阳性2791例(0.29%),HIV确证阳性2605例(0.27%)。HIV抗体筛查检测人份数逐年增加,但确证阳性率整体呈下降趋势,从2010年的0.49%下降至2019年的0.14%(趋势χ^(2)=165.08,P<0.001)。2605例HIV抗体阳性患者中,年龄50岁及以上感染者比例从36.69%升高至54.32%(趋势χ^(2)=26.08,P<0.0001)。未婚感染者比例从12.59%上升至24.07%(趋势χ^(2)=18.37,P<0.0001)。异性性传播成为感染HIV的最主要途径,共计2374例(91.13%),同性性传播156例(5.99%),且呈逐年上升,构成比从1.08%增加到16.67%(趋势χ^(2)=101.21,P<0.0001)。HIV确证阳性者分布于多个就诊科室,主要来源于感染内科(23.72%)、呼吸内科(15.66%)、皮肤性病科(12.86%)、外科(10.13%)和消化内科(7.37%)。结论广西某综合医院就诊患者HIV筛查检测人数逐年增加,但确证阳性率整体呈下降趋势。男男同性性行为(MSM)、年龄≥50岁等就诊人群需作为综合医院开展HIV检测的重点人群,建议综合性医院除了将感染性疾病科、皮肤性病科的就诊者作为HIV检测重点科室外,鼓励在医院内所有科室常规开展主动提供艾滋病检测咨询服务(PITC)工作,尤其是有呼吸道、消化道症状的就诊者可常规进行HIV检测服务。Objective To understand the infection status of HIV and epidemiological characteristics of patients in a general hospital in Guangxi from 2010 to 2019.Methods The HIV test results of outpatients and inpatients in a general hospital in Guangxi from January 2010 to December 2019 were collected,and SPSS22.0 software was used for descriptive statistical analysis.Results From January 2010 to December 2019,a total of 964243 specimens were screened,including 2791 cases(0.29%)that were HIV positive,and 2605 cases(0.27%)that were confirmed as HIV infection.The number of HIV antibody screening tests increased year by year,but the positive confirmation rate of the test was found to have an overall downward trend,from 0.49%in 2010 to 0.14%in 2019(trendχ^(2)=165.08,P<0.001).Among the 2605 HIV-positive patients,the proportion of infected persons aged 50 years and above increased from 36.69%to 54.32%(trendχ^(2)=26.08,P<0.0001).The proportion of unmarried infected persons increased from 12.59%to 24.07%(trendχ^(2)=18.37,P<0.0001).Heterosexual transmission became the most important means of HIV infection,with a total of 2374 cases(91.13%),and 156 cases(5.99%)of homosexual transmission,which was increasing year by year,and the proportion increased from 1.08%to 16.67%(trendχ^(2)=101.21,P<0.0001).HIV-positive patients were distributed in multiple departments,mainly from department of infectious medicine(23.72%),department of respiratory medicine(15.66%),department of dermatology and venereal diseases(12.86%),department of surgery(10.13%)and department of gastroenterology(7.37%).Conclusion The number of patients undergoing HIV screening and testing in a general hospital in Guangxi has increased year by year,but the overall positive rate of confirmed cases has shown a downward trend.Men who have sex with men(MSM),and those aged≥50 years need to be the key groups for HIV testing in general hospitals.It is recommended that general hospitals not only consider patients presenting to department of infectious diseases and department of
