摘 要:目的了解安徽省8~10岁儿童碘营养状况,探讨儿童甲状腺容积和年龄、身高、体重的关系。方法在安徽省选取4个县区(2个山区县和2个平原县),按照《全国碘缺乏病监测方案》要求,采用分层多阶段抽样方法,每个县区按东、西、南、北、中5个方位各抽取1所小学,每所小学抽取8~10岁40名儿童,采集儿童随机1次尿样,测量其身高、体重和甲状腺容积,并对儿童性别、年龄、身高、体重和体质量指数(BMI)与甲状腺容积的关系进行分析。结果本次共调查800名8~10岁儿童,尿碘中位数为247.2μg/L,尿碘<100μg/L的比例为7.4%,≥300μg/L的比例为33.5%。甲状腺肿大儿童有23名,甲肿率为2.9%(23/800),8岁、9岁和10岁组甲肿率分别为2.8%、4.6%和1.1%。随着年龄、身高、体重和BMI的增长,儿童甲状腺容积增大,不同年龄(F=26.738,P<0.001)、身高(P值均小于0.05)、体重(P值均小于0.05)和BMI组儿童(P值均小于0.05)的甲状腺容积差异有统计学意义。儿童年龄、身高、体重与甲状腺容积简单相关,体重和身高与甲状腺容积呈偏相关。结论安徽省8~10岁儿童碘营养超过适宜量水平,甲肿率较低,儿童年龄、身高、体重对甲状腺容积有影响。Objective To investigate the iodine nutrition status of children aged 8-10 years in Anhui Province,and to explore the relationship between thyroid volume and age,height and weight.Methods Firstly,four counties(two mountain counties and two plain counties)were selected in Anhui Province.Then,according to the requirements of the national iodine deficiency disease monitoring program,stratified multi-stage sampling method was adopted to select one primary school from east,west,north,south and middle in each county.Finally,40 children aged 8 to 10 were selected from each primary school.Their urine samples were collected at random once,and their height,weight and thyroid volume were measured,so as to analyze the relationship between gender,age,height,weight and body mass index(BMI)and thyroid volumetam.Results A total of 800 children aged 8-10 years were investigated.The median of urinary iodine was 247.2μg/L,and the proportion of urinary iodine less than 100μg/L and greater than 300μg/L were 7.4%and 33.5%,respectively.Goiter was found in 23children,with a rate of 2.9%(23/800),and 2.8%,4.6%and 1.1%in the 8,9 and 10 year groups,respectively.Thyroid volume increased with the increase of age,height,weight and BMI in children.And the differences in thyroid volume of children of different age(F=26.738,P<0.001),height(P<0.05),weight(P<0.05)and BMI(P<0.05)were statistically significant.Age,height and weight of children were simply correlated with thyroid volume,while the weight and height were partially correlated with thyroid volume.Conclusion The iodine nutrition of children aged 8 to 10 years in Anhui province exceeded the appropriate level and the goiter rate was low.The age,height and weight of children were the influencing factors of thyroid volume.
