桃仁汤联合阿仑膦酸钠治疗中期股骨头缺血性坏死患者的效果

摘　　要：目的:观察桃仁汤联合阿仑膦酸钠治疗中期股骨头缺血性坏死患者的效果。方法:选取2019年1月至2020年1月该院收治的92例中期股骨头缺血性坏死患者进行前瞻性研究,按照随机数字表法分为观察组与对照组各46例。对照组给予阿仑膦酸钠片治疗,观察组在对照组基础上联合桃仁汤治疗,比较两组疗效、髋关节疼痛视觉模拟评分法(VAS)评分、髋关节功能(Harris)评分、血清学指标[白细胞介素-33(IL-33)、红细胞沉降率(ESR)]水平和不良反应发生率。结果:观察组治疗总有效率为95.65%,明显高于对照组的78.26%,差异有统计学意义(P<0.05);治疗后,观察组VAS评分低于对照组,Harris评分高于对照组,差异有统计学意义(P<0.05);治疗后,两组血清IL-33、ESR水平均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:桃仁汤联合阿仑膦酸钠治疗中期股骨头缺血性坏死患者可提高治疗总有效率和Harris评分,以及降低VAS评分和血清学指标水平,效果优于单纯阿仑膦酸钠治疗。Objective:To observe effects of Taoren decoction combined with Alendronate sodium in treatment of patients with midterm avascular necrosis of femoral head.Methods:92 patients with mid-term avascular necrosis of the femoral head admitted to this hospital from January 2019 to January 2020 were selected for the prospective study,and were divided into observation group and control group according to the random number table method,46 cases in each.The control group was treated with Alendronate sodium tablets,while the observation group was treated with Taoren decoction on the basis of that of the control group.The curative effects,the visual analogue scale(VAS)score of hip joint pain,the hip joint function(Harris)score,the serological index levels[interleukin-33(IL-33),erythrocyte sedimentation rate(ESR)]and the incidence of adverse reactions were compared between the two groups.Results:The total effective rate of the observation group was 95.65%,which was significantly higher than the control group of 78.26%,and the difference was statistically significant(P<0.05).After the treatment,the VAS score of the observation group was lower than that of the control group,the Harris score was higher than that of the control group,and the differences were statistically significant(P<0.05).After the treatment,the levels of serum IL-33 and ESR in the two groups were lower than those before the treatment,those of the observation group were lower than those of the control group,and the differences were statistically significant(P<0.05).However,there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions:Taoren decoction combined with Alendronate sodium in the treatment of the patients with mid-term avascular necrosis of the femoral head can improve the total effective rate and the Harris score,and reduce the VAS score and the serological index levels.Moreover,it is superior to single Alendronate sodium treatment.
