摘 要:目的 分析新发感染的艾滋病感染者的CD4^(+)T 淋巴细胞计数水平,探索其影响因素。方法 基于 2019—2021 年医疗机构就诊及全民预防保健体检数据库,筛选出333例HIV新发感染者并测量其CD4^(+)T 淋巴细胞计数水平。采用 t 检验、方差分析比较 CD4^(+)T 淋巴细胞均值、卡方检验比较构成,并用多元线性回归分析探索其影响因素。结果 女性的CD4^(+)T 淋巴细胞水平(358.1±189.9)高于男性(282.5±171.7),差异有统计学意义(t=-3.577,P<0.001);CD4^(+)T 淋巴细胞水平从<40岁年龄组的481.7个/μl下降到≥70岁组的282.6个/μl,年龄差异有统计学意义(F=4.174,P=0.003);不同发病时间的 CD4^(+)T 淋巴细胞水平差异没有统计学意义(P=0.371)。低于 350 个/μl 的比例,从<40 岁年龄组的 28.6%持续上升到≥70 岁组的 80.0%(χ^(2)_(趋势)=14.599,P<0.001);发病时间的构成差异没有统计学意义(χ^(2)_(趋势)=2.098,P=0.147)。多元线性回归分析显示,在调节了性别、年龄因素后,随着发病时间的延长,CD4^(+)T 淋巴细胞水平下降,差异有统计学意义(t=-2.035,P=0.043)。结论 CD4^(+)T 淋巴细胞计数水平与发病时间有关,但要用 CD4^(+)T 淋巴细胞计数判断 HIV 晚发现,需要考虑年龄段的影响。Objective To analyze the CD4^(+)T lymphocyte counts of newly infected AIDS/HIV patients,and to explore the influencing factors.Methods Based on the database of medical institutions and national preventive health care physical examination,about 333 newly infected people with HIV were recruited and their CD4^(+)T lymphocyte count was measured.T test and ANOVA were used to compare the mean value of CD4^(+)T lymphocytes,Chi-square test was used to compare the different of ratio,and the influencing factors were explored by multiple linear regression analysis.Results The level of CD4^(+)T in women(358.1±189.9)was significantly higher than that in men(282.5±171.7),the difference was statistically significant (t=- 3.577,P<0.001);CD4^(+)T levels increased from481.7 in the group of less than 40 years old decreased to 282.6 in the ≥70 years old group,the age difference was statistically significant (F=4.174,P=0.003);There was no significant difference in CD4^(+)T levels at different onset times (P=0.371).The proportion of less than 350/μl increased continuously from 28.6% in the <40 years old group to 80.0% in the ≥70 year old group(χ^(2)_(trend)=14.599,P<0.001);There was no significant difference in the composition of onset time(χ^(2)_(trend)=2.098,P=0.147).Multiple linear regression analysis showed that the level of CD4^(+)T lymphocyte decreased with the extension of onset time after adjusting the factors of gender and age,and the difference was statistically significant (t=-2.035,P=0.043).Conclusions The level of CD4^(+)T lymphocyte count is related to the time of onset,but to judge the late detection of HIV by CD4^(+)T lymphocyte count,we need to consider the influence of age.
