摘 要:目的:探讨磁共振动态增强扫描(dynamic contrast enhanced-magnetic resonance imaging,DCE-MRI)联合弥散加权成像(diffusion weighted imaging,DWI)在肾脏短T_(2)信号肿瘤性病变诊断中的价值。方法:选取2017年1月-2019年6月齐齐哈尔医学院附属第二医院收治的137例经病理证实的肾脏短T_(2)信号肿瘤患者(病变短T_(2)信号范围≥75%)的磁共振影像资料,137例患者中良性病变83例,恶性病变54例,所有患者均进行MR平扫(包括DWI)、DCE-MRI扫描。比较两组病变的表观弥散系数(apparent diffusion coeffecient,ADC)、容量转移常数(K^(trans))、血管外细胞外间隙容积(V_(e))及速率常数(K_(ep))等参数,同时测量受试者操作特征(receiver operator characteristic,ROC)曲线,综合分析以上参数对肾脏短T_(2)信号肿瘤性病变的诊断价值。结果:对比两组的ADC值,恶性组ADC值显著低于良性组(P<0.05)。两组的rADC对比差异无统计学意义(P>0.05)。恶性组的K^(trans)和K_(ep)显著高于良性组(P<0.05);两组的V_(e)对比无显著差异(P>0.05)。ADC、K^(trans)、K_(ep)三者联合应用,对肾脏短T_(2)信号肿瘤性病变的诊断具有更高效能。结论:DCEMRI联合DWI可显著提高肾脏短T_(2)信号肿瘤性病变的诊断准确率,为临床治疗方案的制定提供重要帮助。Objective To investigate the value of dynamic contrast enhanced-magnetic resonance imaging(DCE-MRI)combined with diffusion-weighted imaging(DWI)in the diagnosis of renal short T_(2) signal neoplastic lesions.Methods Magnetic resonance imaging data of 137 patients with pathologically confirmed renal short T_(2) signal tumors(short T_(2) signal range≥75%)admitted to the Second Affiliated Hospital of Qiqihar Medical College from January 2017 to June 2019 were selected 83 of the 137 patients had benign lesions and 54 had malignant lesions.All patients underwent MR plain scan(including DWI)and DCE-MRI scan.Apparent diffusion coefficient(ADC),volume transfer constant(K^(trans)),extracellular space volume(V_(e))and rate constant(K_(ep))were compared between the two groups,and ROC curve was measured to analyze the diagnostic value of these parameters for short T_(2) signal neoplastic lesions of the kidney.Results The ADC value of malignant group was significantly lower than that of benign group(P<0.05).There was no significant difference in rADC between the two groups(P>0.05).K^(trans) and K_(ep) in malignant group were significantly higher than those in benign group(P<0.05).There was no significant difference in Ve between the two groups(P>0.05).The combination of ADC,K^(trans) and K_(ep) has higher diagnostic efficiency for short T_(2) signal neoplastic lesions in kidney.Conclusion DCEMRI combined with DWI can significantly improve the diagnostic accuracy of short T_(2) signal tumor lesions in kidney,providing important help for clinical treatment.
