摘　　要：目的探讨二胎产妇及配偶产后抑郁现状及相关因素。方法采用便利抽样的方法于2019年5月至2020年6月在北京某三级甲等妇产专科医院调查符合研究标准的二胎产妇及配偶。采用问卷调查法收集资料,研究工具包括一般资料调查问卷、爱丁堡产后抑郁量表、社会支持评定量表、中文版知觉压力量表、Kansas婚姻满意感量表。结果210名二胎产妇及其配偶产后抑郁发生率分别为20.95%和17.14%;Logistic回归分析显示:对二胎新生儿性别的期待[OR:4.302,95%CI(1.343,13.784)]、知觉压力[OR:1.228,95%CI(1.137,1.327)]是二胎产妇产后抑郁的影响因素,知觉压力[OR:1.363,95%CI(1.222,1.520)]是配偶产后抑郁的影响因素。结论二胎产妇及其配偶的产后抑郁状况比较严峻,并有明确的相关性,而抑郁受多方面的因素影响。因此,要同时关注二胎产妇及其配偶双方的心理健康,以促进整个家庭的健康和社会的稳定。Objective To explore the status and its influencing factors of postpartum depression in second-birth parturients and their spouses.Methods The convenient sampling method was used to investigate second-birth parturients and their spouses in a tertiary obstetrics and gynecology hospital in Beijing from May 2019 to June 2020.The general situation questionnaire,Edinburgh Postnatal Depression Scale(EPDS),Social Support Rating Scale(SSRS),Chinese version of Perceived Stress Scale(CPSS)and Kansas Marital Satisfaction Scale(KMS)were used to conduct questionnaire surveys.Results The incidence of postpartum depression in 210 second-birth parturients and their spouses were 20.95%and 17.14%.The Logistic regression analysis showed that the expected gender of the second-born newborn[OR:4.302,95%CI(1.343,13.784)],perceived stress[OR:1.228,95%CI(1.137,1.327)]were the influencing factors of postpartum depression in second-birth parturients,while perceived stress[OR:1.363,95%CI(1.222,1.520)]was the influencing factor of spouse's postpartum depression.Conclusion The status of postpartum depression in second-birth parturients and their spouses is very serious.They have a clear correlation,and the depression is affected by many factors.Therefore,the mental health of the parturients and their spouses should be cared,in order to facilitate the health of the entire family and social stability.
