摘　　要：目的探讨肿瘤患者耐高压瓣膜型PICC导管置管后最佳体外留置导管长度,为临床预防非计划拔管护理实践提供依据。方法选择2019年1月至2020年1月在超声引导下结合赛丁格技术行耐高压瓣膜型PICC置管肿瘤患者992例,随机分为观察组494例和对照组498例,对照组置管后导管外露2 cm采用U型固定法,观察组置管后导管外露0 cm采用一字型固定法,比较两组患者导管脱出、非计划拔管发生率。结果观察组导管脱出和非计划拔管发生率明显低于对照组(P<0.05)。结论耐高压瓣膜型PICC导管置管后外露0 cm一字型固定法可显著降低导管脱出率及患者的非计划拔管率。Objective To explore the optimal length of the extracorporeal indwelling catheter after the placement of high pressure resistant valve type PICC catheter in cancer patients,and to provide basis for clinical nursing practice of unplanned extubation prevention.Methods From January 2019 to January 2020,992 tumor patients with high pressure resistant valvular PICC catheter were selected under the guidance of ultrasound combined with Sedinger technique.There were 494 cases in the observation group and 498 cases in the control group.After catheterization,2 cm U-shaped fixation was used for catheter exposure in the control group,and 0 cm in-line fixation was used for catheter exposure in the observation group.The incidence of catheter prolapse and unplanned extubation was compared between the two groups.Results The incidence of catheter prolapse and unplanned extubation in observation group were significantly lower than those in control group(P<0.05).Conclusion High pressure resistant valvular PICC catheter exposed after catheterization can significantly reduce catheter prolapse rate and unplanned extubation rate of patients.
