大学生高血压、肥胖与血清天冬氨酸氨基转移酶异常的相关性分析

摘　　要：目的:探讨大学生高血压、肥胖与血清天冬氨酸氨基转移酶(AST)异常的相关性。方法:测量7515例大学生的血压、身高和体重,并检测血清AST,Logistic回归分析大学生高血压、肥胖对AST异常的影响。结果:大学生AST异常率为5.66%,其中男生9.80%,女生1.78%,差异有统计学意义(P<0.01)。大学生AST异常率均随着血压水平和体重指数的升高而增加,差异均有统计学意义(均P<0.01)。多因素Logistic回归分析提示,超重、肥胖大学生AST异常患病风险分别是体重正常者的2.27倍和6.48倍(均P<0.01);男、女肥胖大学生分别是各自体重正常者的5.76倍、7.78倍(均P<0.01);高血压和正常高值血压大学生AST异常患病风险相对正常血压者的差异没有统计学意义(均P>0.05)。进一步分析发现舒张压升高是AST异常的危险因素(P<0.01)。结论:大学生高血压、超重、肥胖与血清AST异常呈明显正相关,控制血压(主要是舒张压)和体重有助于预防AST异常相关疾病。Objective:To explore the correlation of abnormal serum aspartate aminotransferase(AST)with hypertension and obesity among college freshmen.Methods:There were 7515 freshmen who were measured with blood pressure,height,weight and AST level.Logistic regression was used to analyze the influence of hypertension and obesity on AST abnormality.Results:The rate of abnormal AST was 5.66%.The abnormal AST in males was account for 9.80%,which was significantly higher than 1.78%in females(P<0.01).AST abnormal rate elevated with the increase of body mass index and blood pressure(both P<0.01).Multivariate Logistic regression analysis indicated that the risk of abnormal AST in overweight and obesity freshmen were 2.27 times and 6.48 times of the normal weight,respectively(both P<0.01).The obesity male and females were 5.76 times and 7.78 times of the normal weight,respectively(both P<0.01).There was no statistically significant difference in the risk of abnormal AST among the hypertensive,the high-normal blood pressure and the normal blood pressure(all P>0.05).Further analysis found that elevated diastolic blood pressure was the risk factor for abnormal AST(P<0.01).Conclusion:Overweight,obesity and hypertension could be positively correlated with abnormal AST in freshmen.Controlling blood pressure(mainly diastolic)and body weight would be helpful to prevent abnormal AST related diseases.
