摘　　要：目的:调查家庭医生签约背景下老年慢性病患者社区初级卫生保健服务质量及其影响因素。方法:于2021年1—11月,采用简单随机抽样方法应用初级卫生保健评估工具中文版(PCAT-C),对合肥市4个社区的230例老年慢性病患者进行问卷调查。根据家庭医生签约情况将其分为签约组(n=169)和未签约组(n=61)。结果:调查对象家庭医生签约率为73.48%;签约组老年慢性病患者PCAT-C评分为(77.85±13.29)分,未签约组为(52.72±13.90)分,两组比较,差异有统计学意义(t=12.507,P<0.05);多因素logistic回归分析结果显示,签约家庭医生和近3年有住院经历是初级卫生保健服务评价得分的有利影响因素。结论:签约家庭医生的老年慢性病患者对社区初级卫生保健服务的评分更高,说明其享受到了更高质量的基层卫生服务,因此,应继续推动家庭医生签约服务政策的深化实施。OBJECTIVE To explore the evaluation of primary health care service quality of elderly patients with chronic diseases under the background of family doctor contract and its influencing factors.METHODS A questionnaire was administered to 230 elderly patients with chronic diseases in four communities in Hefei from January to November 2021 using a simple random sampling method applying the Primary Care Assessment Tool Chinese version(PCAT-C).They were divided into a contracted group(n=169)and an uncontracted group(n=61)based on the contracted status of the family doctor.RESULTS The signing rate of family doctors in the survey is 73.48%.The total score of PCAT-C assessment for elderly patients with chronic diseases was(77.85±13.29)in the contracted group and(52.72±13.90)in the non-contracted group.There were significant differences in scores of the PACT-C of the two groups(t=12.507,P<0.05).The logistic regression analysis showed that contracting a family doctor and having been hospitalised in the last 3 years were favourable influencing factors in the evaluation scores of primary health care services.CONCLUSION Elderly patients with chronic diseases who signed up with family doctors rated community primary health care services higher,indicating that they enjoyed higher quality primary health care services,therefore,the deepening of the implementation of the family doctor contract service policy should continue to be promoted.
