蔗糖铁注射液联合促红细胞生成素治疗肾性贫血的临床研究

摘　　要：目的 探讨蔗糖铁注射液联合促红细胞生成素治疗肾性贫血的临床疗效。方法 筛选2019年4月至2021年4月在本院收治的肾性贫血的患者,运用随机数字表法将选取的126例目标患者分成对照组和试验组,每组63例。对照组患者采用蔗糖铁治疗,试验组在对照组的基础上联合促红细胞生成素治疗。对比2组患者的临床疗效、治疗前后血红蛋白(Hb)、红细胞比容(HCT)、血清铁蛋白(SF)水平指标、生活质量量表(GQOL-74)评分、红细胞代谢能力指标以及药物不良反应的发生情况。结果 治疗后,试验组和对照组患者的治疗有效率分别为95.24%(60例/63例)和84.13%(53例/63例),Hb水平分别为(109.97±10.83)和(90.85±8.67)g·L^(-1),HCT水平分别为(32.88±2.92)%和(26.46±1.04)%,SF水平分别为(444.00±63.20)和(412.01±68.05)U·L^(-1),心理功能评分分别为(83.99±6.51)和(69.57±7.78)分,社会功能评分分别为(85.24±6.73)和(73.65±6.67)分,躯体功能评分分别为(80.10±4.52)和(70.57±6.01)分,物质生活评分分别为(87.21±5.96)和(72.70±4.53)分,红细胞内钙浓度(Ca^(2+))指标水平分别为(0.94±0.09)和(1.58±0.12)μg·mL^(-1),红细胞长、短轴计算比值(L/W)指标水平分别为(2.87±0.36)和(2.64±0.28),上述所有指标差异均有统计学意义(均P<0.05),2组患者药物不良反应有恶心呕吐、高血压以及发热等,试验组药物不良反应发生率为4.76%(3例/63例),对照组药物不良反应发生率15.87%(10例/63例)(P<0.05)。结论 蔗糖铁注射液联合促红细胞生成素治疗肾性贫血的临床效果较好,患者贫血指标得到调节,有效增强机体细胞应激能力,药物不良反应的发生率低,提升患者生活质量。Objective To investigate the clinical efficacy of iron sucrose injection combined with erythropoietin in the treatment of renal anemia.Methods The patients with renal anemia who were treated in our hospital from April 2019 to April 2021 were screened,126 target patients were selected to participate in this study,and they were divided into control group and treatment group by random number table method.There are 63 cases in each group.The patients in the control group were treated with iron sucrose,and the patients in the treatment group were treated with erythropoietin on the basis of the control group.The clinical efficacy,hemoglobin(Hb),hematocrit(HCT),serum ferritin(SF) level indexes,quality of life scale(GQOL-74) score,red blood cell metabolic capacity indexes and adverse drug reactions before and after treatment were compared between the two groups.Results After treatment,the effective rates of the treatment group and control group were 95.24%( 60 cases/63 cases) and 84.13%( 53 cases/63 cases),and the Hb levels were( 109.97 ± 10.83) and( 90.85 ± 8.67)g·L^(-1),respectively;HCT levels were( 32.88 ± 2.92) % and( 26.46 ± 1.04) %;SF levels were( 444.00 ± 63.20)and( 412.01 ± 68.05) U · L^(-1);psychological function scores of the two groups were( 83.99 ± 6.51) and( 69.57 ± 7.78) points,respectively;social function scores were( 85.24 ± 6.73) and( 73.65 ± 6.67) points;physical function scores were( 80.10 ± 4.52) and( 70.57 ± 6.01) and material life scores were( 87.21 ± 5.96) and( 72.70 ± 4.53) points;the levels of Ca^(2+) were( 0.94 ± 0.09) and( 1.58 ± 0.12) μg·m L^(-1),respectively;and the levels of L/W were( 2.87 ± 0.36) and( 2.64 ± 0.28),respectively.All the above indicators differences were statistically significant( P < 0.05),adverse drug reactions in the two groups included nausea,vomiting,hypertension and fever,etc.The incidence of adverse drug reactions in the treatment group was 4.76%( 3 cases/63 cases),and the incidence of adverse drug reactions in the control group was 15.87%( 10 case
