摘  要:目的探讨使用自制输尿管抱镜救援装置解除术中输尿管抱镜的有效性及安全性。方法回顾性分析2016年1月-2021年1月北京大学第三医院延庆医院1622例行输尿管镜术患者的临床资料,其中18例患者应用输尿管抱镜救援装置(专利号:ZL 201521086088.X)协助输尿管镜及输尿管通道鞘退出。术后放置F5或F6双“J”管,定期随访观察术后有无并发症发生。结果18例患者术中使用了输尿管抱镜救援装置,其中男性13例,女性5例,平均年龄47.5(45±10.5)岁;左侧6例,右侧12例。18例应用自制输尿管抱镜救援装置者均顺利地将嵌顿的输尿管镜及输尿管通道鞘退出,成功率为100%。术中有5例输尿管口撕裂、2例膀胱颈黏膜损伤,同救援鞘置入有关,围术期再无其他并发症发生。术后放置F5或F6双“J”管4~12周,平均随访9(4~12)个月,原发疾病恢复良好,输尿管无狭窄梗阻,无肾、输尿管积水。结论自制输尿管抱镜救援装置解除输尿管镜术中出现的输尿管抱镜等安全、有效、可行。Objective To investigate the effectiveness and safety of using a self-made ureteroscopic incarceration rescue device to relieve intraoperative ureteroscopic incarceration.Methods The clinical data of 1622 patients undergoing ureteroscopy in our hospital during Jan.2016 and Jan.2021 were retrospectively analyzed,including 18 cases assisted with our ureteroscopic incarceration rescue device(patent number:ZL201521086088.X).After surgery,F5 or F6 double"J"tubes were placed,and regular follow-up was conducted to observe the complications.Results The ureteroscopic incarceration rescue devices(6 on the left and12 on the right)were used in18 patients,including 13 males and5 females,with an average age of 47.5(45.0±10.5).In all 18 patients,the rescue device and ureteral channel sheath were removed successfully,with a success rate of100%.Ureteral orifice tear occurred in5 cases,and bladder neck mucosa injury in2 cases,which were related to the rescue device;no other complications were observed.After surgery,F5 or F6 double"J"tubes were placed for 4 to 12 weeks.During an average follow-up of 9(4 to 12)months,the primary disease recovered well;there was no stenosis obstruction of the ureters,and no hydrops of the kidneys and ureters.Conclusion The self-made ureteroscopic incarceration rescue device is safe,effective and feasible to relieve ureteroscopic incarceration that occurs during ureteroscopy.
