摘 要:目的 关于葡萄籽原花青素(GSPE)与糖尿病牙周炎的研究较少。文中通过建立糖尿病牙周炎大鼠模型,探究GSPE对大鼠血糖、血清脂联素、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)及牙周组织炎性状态的影响。方法 选取50只雄性SD大鼠随机数字表法等分为5组,即对照组、单纯牙周炎组、糖尿病牙周炎组、单纯牙周炎GSPE干预组、糖尿病牙周炎GSPE干预组。单纯牙周炎GSPE干预组和糖尿病牙周炎GSPE干预组给予GSPE灌胃(100 mg/kg, 1次/d),对照组、单纯牙周炎组和糖尿病牙周炎组给予等量等渗盐水灌胃,连续8周。监测大鼠体重及血糖变化;检测大鼠第一磨牙牙周指数(牙龈指数和龈沟出血指数)及牙槽骨吸收情况;HE染色观察大鼠牙周组织病理改变;酶联免疫吸附(ELISA)法测定大鼠血清脂联素、TNF-α、和IL-6水平。结果 糖尿病牙周炎组和糖尿病牙周炎GSPE干预组大鼠体重较对照组明显减轻(P<0.05),给药8周后,糖尿病牙周炎组大鼠体重[(251±19.2)g]较糖尿病牙周炎GSPE干预组[(278±24.8)g]明显减轻(P<0.05)。糖尿病牙周炎组和糖尿病牙周炎GSPE干预组大鼠血糖较对照组显著升高(P<0.05),糖尿病牙周炎组大鼠血糖明显高于糖尿病牙周炎GSPE干预组(P<0.05)。单纯牙周炎组和糖尿病牙周炎组牙周指数较对照组升高(P<0.05);糖尿病牙周炎组牙周指数和牙槽骨吸收值较单纯牙周炎组升高(P<0.05);糖尿病牙周炎GSPE干预组牙周指数和牙槽骨吸收值较糖尿病牙周炎组明显降低(P<0.05)。糖尿病牙周炎组和糖尿病牙周炎GSPE干预组血清脂联素[(16.64±0.84、19.50±1.16)ng/mL]水平明显低于对照组[(24.78±1.57)ng/mL](P<0.05);糖尿病牙周炎GSPE干预组血清脂联素水平较糖尿病牙周炎组显著升高(P<0.05)。单纯牙周炎组和糖尿病牙周炎组血清TNF-α[(20.26±1.87、25.01±1.33)ng/mL]及IL-6[(11.32±1.53、15.69±0.56)ng/mL]水平明显高于对照组[(14.74±0.89)Objective There are few studies on grape seed proanthocyanidins extract(GSPE) and diabetic periodontitis. In this paper, the rat model of diabetic periodontitis was established to investigate the effects of GSPE on blood glucose, serum adiponectin, tumor necrosis factor α(TNF-α), interleukin 6(IL-6) and periodontal inflammation. Methods Fifty male SD rats were randomly divided into 5 groups: control group, periodontitis group, diabetic periodontitis group, periodontitis GSPE intervention group, diabetic periodontitis GSPE intervention group. The two intervention groups were given GSPE gavage(100 mg/kg, once a day), while the control group, the periodontitis group and the diabetic periodontitis group were given the same amount of isotonic saline gavage for 8 consecutive weeks. The changes of body weight and blood glucose were monitored;The periodontal index(gingival index and sulcus bleeding index) and alveolar bone resorption of the first molar were detected;HE staining was used to observe the pathological changes of periodontal tissue;The levels of serum adiponectin, TNF α and IL 6 were determined by enzyme-linked immunosorbent assay(ELISA). Results The body weight of rats in diabetic periodontitis group and diabetic periodontitis GSPE intervention group was significantly lower than that in control group(P<0.05). After 8 weeks of treatment, the body weight of rats in diabetic periodontitis group [(251±19.2) g] was significantly lower than that in diabetic periodontitis GSPE intervention group [(278±24.8) g](P<0.05). The blood glucose of rats in diabetic periodontitis group and diabetic periodontitis GSPE intervention group was significantly higher than that in control group(P<0.05), and the blood glucose of rats in diabetic periodontitis group was significantly higher than that in diabetic periodontitis GSPE intervention group(P<0.05).
