摘 要:目的探究血清C-肽和糖化血红蛋白(glycated hemoglobin A1c,HbA1c)联合检测对糖尿病的诊断价值。方法选取2021年6—12月启东市人民医院内分泌科收治的138例糖尿病患者作为研究组,另纳入同期健康体检者50名作为参照组。对比两组血清C-肽、空腹2 h血糖、餐后2 h血糖水平以及糖化血红蛋白(HbA1c)水平,评估两项指标单一与联合检测的诊断价值。结果研究组血清C-肽值(1.29±0.45)ng/mL低于参照组(3.12±0.56)ng/mL,空腹血糖(9.64±1.53)mmol/L、餐后2 h血糖(12.16±1.83)mmol/L以及糖化血红蛋白(9.62±2.23)%高于参照组(5.21±1.45)mmol/L、(7.73±1.36)mmol/L、(5.03±1.47)%,差异有统计学意义(P<0.001);血清C-肽与糖化血红蛋白单一检测准确度(84.78%、86.23%)与灵敏度(83.33%、84.78%)均低于联合检测(94.93%、100.00%);漏诊率(14.49%、12.32%)均高于联合检测(0.00%),差异有统计学意义(P<0.05);单一检测与联合检测的特异度对比,差异无统计学意义(P>0.05)。结论应用血清C-肽与糖化血红蛋白联合检测的方式诊断糖尿病的效果理想,能有效提高临床检测的准确性。Objective To explore the diagnostic value of combined detection of serum C-peptide and glycosylated hemoglobin(HbA1c)in diabetes.Methods A total of 138 diabetic patients admitted to the Endocrinology Department of Qidong People’s Hospital from June to December 2021 were selected as the study group,and another 50 healthy people who underwent physical examinations during the same period were collected as the reference group.The serum C-peptide,fasting and 2 h postprandial blood glucose levels and glycated hemoglobin HbA1c levels were compared between the two groups to evaluate the diagnostic value of single and combined detection of the two indicators.Results The serum C-peptide value of the study group(1.29±0.45)ng/mL was lower than that of the reference group(3.12±0.56)ng/mL;fasting blood glucose(9.64±1.53)mmol/L,2 h postprandial blood glucose(12.16±1.83)mmol/L and the levels ofglycosylated hemoglobin(9.62±2.23)%were higher than those of the reference group(5.21±1.45)mmol/L,(7.73±1.36)mmol/L,(5.03±1.47)%,the difference was statistically significant(P<0.001).The single detection accuracy(84.78%,86.23%)and sensitivity(83.33%,84.78%)of serum C-peptide and glycosylated hemoglobin were lower than the combined detection(94.93%,100.00%);the missed diagnosis rate(14.49%,12.32%)were both higher than the combined detection(0.00%),the difference was statistically significant(P<0.05);there was no statistically significant difference in specificity between single detection and combined detection(P>0.05).Conclusion The combined detection of serum C-peptide and glycosylated hemoglobin has an ideal effect in the diagnosis of diabetes,and it can effectively improve the accuracy of clinical detection.
