持续胰岛素泵输注在1型儿童糖尿病临床干预中的应用效果

摘  要:目的探讨持续胰岛素泵输注在1型儿童糖尿病临床干预中的应用效果。方法选择2018年3月—2021年12月间在福建医科大学附属协和医院接受治疗的58例1型糖尿病患儿作为研究对象,参照随机数表法分为多次胰岛素注射组、持续胰岛素泵组,各29例。多次胰岛素注射组患儿接受多次皮下胰岛素注射,持续胰岛素泵组患儿接受持续胰岛素泵输注,对比两组患儿治疗前后糖脂代谢水平、治疗相关不良反应、治疗满意度。结果治疗3个月后,持续胰岛素泵组FPG、HbA1c、LDL-C水平分别为(4.65±0.78)mmol/L、(6.01±0.77)%、(2.50±0.65)mmol/L,低于多次胰岛素注射组患儿(5.74±0.69)mmol/L、(6.85±0.85)%、(3.09±0.48)mmol/L,差异有统计学意义(P<0.05)。治疗期间,持续胰岛素泵组患儿的低血糖、酮症酸中毒发生率分别为3.45%、0.00%,低于多次胰岛素注射组患儿的24.14%、13.79%,差异有统计学意义(P<0.05)。两组患儿的心血管病、肾脏病发生率比较,差异无统计学意义(P>0.05)。治疗3个月后,持续胰岛素泵组患儿家属的总满意率为93.10%,高于多次胰岛素注射组的72.41%,差异有统计学意义(P<0.05)。结论1型糖尿病患儿采用持续胰岛素泵治疗,较多次胰岛素注射在优化糖脂代谢、减少严重不良反应发生方面更具优势,且有更高的治疗满意率。Objective To investigate the application effect of continuous insulin pump infusion in the clinical intervention of type 1 children with diabetes.Methods A total of 58 children with type 1 diabetes who were treated in Fujian Medical University Affiliated Union Hospital,Fuzhou from March 2018 to December 2021 were selected as the research subjects.According to the random number table method,divided into multiple insulin injection group and continuous insulin pump group,29 cases in each group.The children in the multiple insulin injection group received multiple subcutaneous insulin injections,and the children in the continuous insulin pump group received continuous insulin pump infusion.The differences in the levels of glucose and lipid metabolism,treatment-related adverse reactions,and treatment satisfaction were compared between the two groups before and after treatment.Results After 3 months of treatment,the levels of FPG,HbA1c and LDL-C in the continuous insulin pump group were(4.65±0.78)mmol/L,(6.01±0.77)%,and(2.50±0.65)mmol/L,respectively,lower than the multiple insulin injection group(5.74±0.69)mmol/L,(6.85±0.85)%,(3.09±0.48)mmol/L,the differences were statistically significant(P<0.05).During treatment,the incidences of hypoglycemia and ketoacidosis in the continuous insulin pump group were 3.45%and 0.00%,respectively,which were lower than 24.14%and 13.79%in the multiple insulin injection group,and the differences were statistically significant(P<0.05).There was no significant difference in the incidence of cardiovascular disease and kidney disease between the two groups(P>0.05).After 3 months of treatment,the total satisfaction rate of the family members of the children in the continuous insulin pump group was 93.10%,which was higher than that in the multiple insulin injection group(72.41%),and the difference was statistically significant(P<0.05).Conclusion Children with type 1diabetes are treated with continuous insulin pump,and more insulin injections have more advantages in optimizing glucose and
