摘 要:目的研究低碳生酮饮食对2型糖尿病(type 2 diabetes mellitus,T2DM)糖脂代谢、血尿酸(uric acid,UA)代谢、体质指数(body mass index,BMI)及腰围的影响。方法选择2018年6月—2020年9月广东省深圳市宝安区中心医院收治的80例T2DM患者,通过电脑随机数表法分为干预组及对照组,各40例。对照组予以普通糖尿病膳食干预,干预组则予以低碳生酮饮食干预。分析两组糖脂代谢、UA代谢、BMI及腰围变化情况、干预前后胰岛素分泌功能、并发症发生情况的差异。结果干预后,干预组各项糖脂代谢指标及血尿酸水平均低于对照组,差异有统计学意义(P<0.05)。干预组BMI及腰围均低于对照组,差异有统计学意义(P<0.05)。干预组空腹胰岛素(FINS)及胰岛素原(TI)水平均高于对照组,差异有统计学意义(P<0.05)。干预组并发症总发生率低于对照组,差异有统计学意义(P<0.05)。结论低碳生酮饮食可明显改善T2DM患者的糖脂代谢、血尿酸代谢,同时可明显降低BMI及腰围,改善胰岛素分泌功能,降低并发症发生风险。Objective To study the effect of low-carb ketogenic diet on glucose and lipid metabolism,serum uric acid(UA)metabolism,body mass index(BMI)and waist circumference in type 2 diabetes mellitus(T2DM).Methods A total of 80 patients with T2DM admitted to Bao’an District Central Hospital of Shenzhen,Guangdong Province from June 2018 to September 2020 were selected and divided into intervention group and control group by computer random number table method,with 40 cases in each group.The control group was given general diabetic diet intervention,while the intervention group was given low carb ketogenic diet intervention.The differences in glucose and lipid metabolism,UA metabolism,BMI and waist circumference,insulin secretion function before and after intervention,and complications were analyzed between the two groups.Results After intervention,various glucose and lipid metabolism indexes and blood uric acid levels in the intervention group were lower than those in the control group,the difference was statistically significant(P<0.05).The BMI and waist circumference of the intervention group were lower than those of the control group,the difference was statistically significant(P<0.05).The fasting insulin(FINS)and proinsulin(TI)levels in the intervention group were higher than those in the control group,the difference was statistically significant(P<0.05).The total incidence of complications in the intervention group was lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion Low-carbon ketogenic diet can significantly improve glucose and lipid metabolism and blood uric acid metabolism in patients with T2DM.At the same time,it can significantly reduce BMI and waist circumference,improve insulin secretion function,and reduce the risk of complications.
