摘　　要：目的探究第一次激光时的黄斑颞侧视网膜两排激光光凝对全视网膜激光光凝的糖尿病性视网膜病变(diabetic retinopathy,DR)患者影响效果。方法选取厦门市仙岳医院2020年5月—2022年4月收治的DR患者92例,将其随机均分为两组,分别进行4次全视网膜激光光凝,研究组患者于第一次激光时接受黄斑颞侧视网膜两排激光光凝,其余治疗与参照组患者保持一致,对所有患者进行术后随访,对比两组患者治疗效果、新生血管水平、视力、眼压以及黄斑中心凹视网膜厚度(CMT)。结果治疗前,两组患者新生血管水平、视力、眼压及CMT指标均差异无统计学意义(P>0.05);治疗后,研究组患者治疗有效率相较于参照组患者显著提升,血管生成素2(Ang-2)、血管内皮生长因子(VEGF)指标相较于参照组患者显著下降,而视力及CMT指标优于参照组,差异有统计学意义(P<0.05)。结论对于DR患者而言,第一次激光时的黄斑颞侧视网膜两排激光光凝的应用意义显著,保障恢复效果。Objective To investigate the effect of two-row laser photocoagulation on the macular and temporal retina during the first laser photocoagulation on diabetic retinopathy(DR)patients with panretinal laser photocoagulation.Methods A total of 92 DR patients admitted to Xiamen Xianyue Hospital from May 2020 to April 2022 were selected and randomly divided into two groups,who underwent 4 times of panretinal laser photocoagulation.The patients in the study group received two-row laser photocoagulation on the temporal side of the macular retina during the first laser.The rest of the treatment was the same as that of the patients in the reference group,and all patients were followed up after surgery.The treatment effect,neovascularization level,visual acuity,intraocular pressure and foveal retinal thickness(CMT)were compared between the two groups.Results There was no statistically significant difference in the level of neovascularization,visual acuity,intraocular pressure and CMT between the two groups before treatment(P>0.05).After treatment,the treatment effective rate of the patients in the study group was significantly improved compared with the patients in the reference group,and the Ang-2 and VEGF indexes were significantly decreased compared with the patients in the reference group.The visual acuity and CMT indexes were better than the reference group,and the difference was statistically significant(P<0.05).Conclusion For DR patients,the application of tworow laser photocoagulation on the temporal side of the macular and temporal retina during the first laser is of significant significance,guarantees the recovery effect.
