42例小儿会阴部皮肤损伤临床特征分析

摘　　要：目的总结42例小儿会阴部皮肤损伤的临床特征及修复方法。方法回顾性分析2019-01—2021-01经郑州大学第一附属医院收治的42例会阴部皮肤损伤患儿的临床资料,复习国内外相关文献归纳并总结。结果Ⅰ°20例行保守治疗,Ⅱ°~Ⅳ°22例行个体化手术治疗。Ⅱ°原位缝合术6例、包皮环切术5例、改良包皮环切+系带成形术2例、原位缝合+改良包皮环切+系带成形术1例,Ⅲ°行阴茎包皮皮瓣转移阴茎成形术2例、嵌顿包皮松解+包皮环切术2例、阴囊中缝皮肤岛状皮瓣修补术1例,Ⅳ°行阴囊中缝皮肤岛状皮瓣修补术1例、复合皮瓣(左侧带蒂阴囊皮瓣修补+包皮皮瓣转移)成形术+双侧睾丸探查1例、背侧包皮皮瓣转移修补1例。随访时间为6个月~1 a。患儿阴茎阴囊外形、排尿及阴茎勃起功能均正常,41例未见明显其他异常体征及症状出现。结论对于初步诊断为小儿会阴部皮肤损伤的患儿,对其进行损伤分度,有利于其修复方案的制定。Objective To summarize the clinical characteristics and repair methods of 42 cases of pediatric perineal skin injuries.Methods The clinical data of 42 children with perineal skin injury in the first affiliated Hospital of Zhengzhou University from January 2019 to January 2021 were collected retrospectively,and relevant literature domestic and foreign was summarized and reviewed.Results Totally 20 cases of Ⅰ°were treated conservatively.22 cases of Ⅱ°-Ⅳ°were treated with individualized surgery.And among the 14 children of Ⅱ°,6 cases were treated with in-situ suture,5 cases with circumcision,2 cases with modified circumcision&tetherplasty,1 case with in-situ suture&modified circumcision&tetherplasty.And among the 5 children of Ⅲ°,2 cases were treated with penile foreskin flap penileplasty,2 cases with embedded foreskin release&circumcision,1 case with scrotal mid-suture skin island flap repair.And among the 3 children of Ⅳ°,1 case was treated with scrotal mid-suture skin island flap,1 case with composite flap(left-sided tipped scrotal flap&foreskin flap)&bilateral testicular exploration,1 case with dorsal foreskin flap.The follow-up period of these children was from 6 months to 1 year.After repairing,the scrotal shape,urination and erectile function of the penis of the children were normal,and 41 cases with no obvious other abnormal signs and symptoms.Conclusion For the children who are initially diagnosed as perineal skin injury,the injury grading is conducive to the formulation of repair plan.
