HLA-DQ、HLA-DR基因多态性与广西南宁居民肠易激综合征的相关性研究

摘　　要：目的:探讨HLA-DQ、HLA-DR基因多态性与广西南宁市社区居民肠易激综合征(IBS)的相关性。方法:分别收集广西南宁市社区居民IBS患者、健康志愿者外周血标本102例、108例,常规提取DNA,采用序列特异性引物聚合酶链反应(PCR-SSP)进行HLA-DQ、HLA-DR基因多态性鉴定,采用统计学方法分析IBS患者组与健康对照组HLA-DQ、HLA-DR的基因型和基因分布频率,并对两组间基因分布频率进行比较。结果:(1)IBS患者组HLA-DQ6基因表达频率高于健康对照组(44.1%vs29.6%,OR=1.875),而HLA-DQ2基因表达频率低于健康对照组(14.7%vs 28.7%),差异有统计学意义(P<0.05);(2)IBS患者组HLA-DR13基因表达频率高于健康对照组(17.6%vs 7.4%,OR=2.679),而HLA-DR17基因表达频率低于健康对照组(8.8%vs19.4%),差异有统计学意义(P<0.05)。其余等位基因在两组间的基因频率经比较差异无统计学意义(P>0.05)。结论:广西南宁居民IBS的遗传易感性可能与HLA-DQ、HLA-DR基因多态性相关,HLA-DQ6、HLA-DR13基因可能是广西南宁居民IBS的易感基因,而HLA-DQ2、HLA-DR17基因可能具有保护作用。Objective:To investigate correlation between HLA-DQ and HLA-DR gene polymorphisms and irritable bowel syndrome(IBS)in residents of Nanning,Guangxi.Methods:Peripheral blood samples were collected from 102 IBS patients and 108 healthy volunteers in Nanning,Guangxi,and DNA was extracted routinely,HLA-DQ and HLA-DR gene polymorphisms were identified by sequence specific primer polymerase chain reaction(PCR-SSP),to comprehend genotype and distribution frequency of HLA-DQ and HLA-DR in IBS patients and healthy controls,and compared gene distribution frequency between two groups.Results:(1) Frequency of HLA-DQ6 gene expression in IBS patients group was higher than that in healthy control group(44.1% vs 29.6%,OR=1.875),while frequency of HLA-DQ2 gene expression in IBS patients group was lower than that in healthy control group(14.7% vs 28.7%),the difference was statistically significant(P<0.05).(2)Frequency of HLA-DR13 gene expression in IBS patients group was higher than that in healthy control group(17.6% vs 7.4%,OR=2.679),while frequency of HLA-DR17 gene expression in IBS patients group was lower than that in healthy control group(8.8% vs 19.4%),the difference was statistically significant(P<0.05). There was no significant difference in frequency of other alleles between two groups(P>0.05).Conclusion:Genetic susceptibility of IBS in Nanning residents of Guangxi may be related to HLA-DQ and HLA-DR gene polymorphisms,HLA-DQ6 and HLA-DR13 genes may be the susceptibility genes of IBS,and HLA-DQ2 and HLA-DR17 genes may have protective effect.
