主刺中膂俞治疗脊髓损伤后尿潴留的临床观察

摘　　要：目的观察主刺中膂俞对脊髓损伤后尿潴留的治疗效果。方法将80例脊髓损伤后尿潴留患者随机分为对照组与治疗组各40例。两组均给予膀胱功能训练,治疗组在对照组基础上主刺中膂俞,配穴:次髎、肾俞、小肠俞、大肠俞、膀胱俞。每日1次,每周5次,连续4周。统计并分析治疗前后患者膀胱残余尿量、尿动力学指标(包括膀胱容量、最大尿流速、膀胱内压)、生活自理能力。结果4周后,治疗组膀胱残余尿量、尿动力学指标、生活自理能力相对于对照组明显好转(P<0.05),总有效率92.50%,高于对照组的75.00%(P<0.05)。结论主刺中膂俞可有效提高临床疗效,改善脊髓损伤后尿潴留患者的膀胱残余尿量、尿动力学指标、生活自理能力,进一步提高患者的生活质量。Objective:To explore the therapeutic effect of the main acupuncture of Zhonglyushu on urinary retention after spinal cord injury.Methods:A total of 80 patients with urinary retention after spinal cord injury were selected and randomly divided into a control group and a treatment group,40 cases in each group.Both groups were given bladder function training.On the basis of the control group,the treatment group received main acupuncture on Zhonglyushu,with points combination of Ciliao,Shenshu,Xiaochangshu,Dachangshu and Pangguangshu.The treatment was conducted 1 time a day,5 times a week for 4 consecutive weeks.The bladder residual urine volume,urodynamic indexes(including bladder capacity,maximum urine flow rate,intravesical pressure),self-care ability and clinical effects of the two groups were counted and analyzed before and after treatment.Results:After 4 weeks,the bladder residual urine volume,urodynamic indexes and self-care ability of the treatment group were significantly improved compared with those of the control group(P<0.05).The effective rate of patients in the treatment group was 92.50%,and the effective rate of patients in the control group was 75.00%,and the difference was statistically significant(P<0.05).Conclusion:Main acupuncture on Zhonglyushu can effectively improve bladder residual urine volume,urodynamic indexes,and self-care ability in patients with urinary retention after spinal cord injury,improve clinical curative effect,and further improve the living standard of patients,which has provided a new therapeutic target for traditional Chinese medicine treatment of urinary retention after spinal cord injury.
