补肾活血方对不明原因复发性流产未孕患者血清PP14、IL-17及IL-23影响的研究

摘　　要：目的 通过临床试验,以人胎白蛋白(PP14)蛋白和白介素17(IL-17)、白介素23(IL-23)为观察指标,观察补肾活血方在孕前治疗复发性流产(RSA)的疗效,采用酶联免疫吸附法(ELISA)测定PP14蛋白和IL-17、IL-23用药干预前后的表达情况。结合中医治疗滑胎的理念“未病先防”“预培其损”,探究PP14蛋白和IL-17、IL-23的表达情况与不明原因型复发性流产(URSA)的关系。方法 选择30例未孕URSA肾虚血瘀型患者作为试验组,29例正常未孕女性作为对照组。两组均于月经相分泌晚期采血后入组,3个月后分泌晚期再次采血。此期间观察组口服补肾活血方。在相应时间用ELISA法检测正常未孕女性和补肾活血方干预后的URSA肾虚血瘀型未孕女性外周血中PP14蛋白和IL-17、IL-23水平。结果 补肾活血方可使URSA未孕女性外周血中PP14表达水平升高,IL-17及IL-23水平降低。且三者之间具有显著相关性。结论 未孕状态下,PP14、IL-17及IL-23与URSA相关,可能是由于三者的表达失衡导致了URSA发生。补肾活血方可能通过提高未孕女性血清PP14含量,同时降低IL-17及IL-23的表达,达到改善URSA的效果。Objective Clinical experiments were conducted to explore the efficacy of Bushen Huoxue Prescription(补肾活血方) in the treatment of recurrent spontaneous abortion(RSA) before pregnancy by using PP14 protein and interleukin 17(IL-17) and interleukin 23(IL-23) as the observation indexes.The expressions of PP14 protein,IL-17 and IL-23 before and after the intervention were determined by enzyme-linked immunosorbent assay(ELISA).The relationship between their expressions and the RSA was investigated based on the concept of “prevention before disease” and “repairing the damage in advance”in traditional Chinese medicine(TCM) for the treatment of sliding fetus.Methods Thirty non pregnant patients with unexplained recurrent spontaneous abortion(URSA) of kidney deficiency and blood stasis were selected as the observation group,and 29 normal non pregnant women were as the control group.The blood of both groups was collected at the late stage of menstrual secretion when they were recruited and 3 months later.During this period,the observation group took Bushen Huoxue Prescription.In the meantime,the levels of PP14 protein,IL-17 and IL-23 in peripheral blood were detected by ELISA for both the non pregnant URSA women with kidney deficiency and blood stasis as well as the normal non pregnant women.Results Bushen Huoxue Prescription can increase the PP14 expression level and decrease the levels of IL-17 and IL-23 in peripheral blood of the non pregnant women with URSA,among which significant correlations existing.Conclusion In the non pregnant state,PP14,IL-17 and IL-23 are related to URSA,which may be caused by the unbalanced expressions of the three factors.Bushen Huoxue Prescription can increase the level of PP14,and decrease the levels of IL-17 and IL-23,which are helpful for the treatment of URSA.
