逍遥散加减联合脉冲染料激光治疗中重度痤疮效果及对血清Th1/Th2型细胞因子的作用

摘　　要：目的 探讨逍遥散加减联合脉冲染料激光治疗中重度痤疮效果及对患者血清辅助性T细胞1(Th1)/Th2型细胞因子的作用。方法 选取医院中重度痤疮患者110例,简单随机化分为联合组(55例)、对照组(55例)。对照组予以脉冲染料激光,联合组予以逍遥散加减联合脉冲染料激光,均治疗6周。比较两组疗效、治疗前后痤疮症状评分、皮肤病生活质量指数(DLQI)评分、炎性因子指标[肿瘤坏死因子-α(TNF-α)、白细胞计数(WBC)、C反应白蛋白(CRP)]、免疫功能指标(CD^(+)_(3)、CD^(+)_(4)、CD^(+)_(4)/CD^(+)_(8))、Th1/Th2细胞因子[干扰素-γ(IFN-γ)、白细胞介素-4(IL-4)]水平。结果 联合组治疗后总有效率(92.73%,51/55)高于对照组(78.18%,43/55)(P<0.05)。联合组治疗后脓疱、粉刺、炎性丘疹及结节囊肿评分、DLQI评分低于对照组(P<0.05)。联合组治疗后血清TNF-α、WBC、CRP、IL-4水平低于对照组,IFN-γ水平、CD^(+)_(3)、CD^(+)_(4)、CD^(+)_(4)/CD^(+)_(8)水平高于对照组(P<0.05)。结论 逍遥散加减联合脉冲染料激光治疗中重度痤疮能有效调节细胞免疫、体液免疫,从而降低免疫炎性反应,有效减轻痤疮症状,提高患者生活质量,效果显著。Objective To investigate the effect of Modified Xiaoyao Powder(逍遥散加减) combined with pulsed dye laser in the treatment of moderate and severe acne and its effect on serum helper T cell 1(Th1)/Th2 type cytokines.Methods A total of 110 patients with moderate to severe acne in the hospital were selected and divided into a combination group(55 cases) and a control group(55 cases) according to simple randomization.The control group was given pulsed dye laser,and the combination group was given Modified Xiaoyao Powder combined with pulsed dye laser.Both groups were treated for 6 weeks.The efficacy,the acne symptom score,dermatology life quality index(DLQI) score,the levels of inflammatory factor indexes[tumor necrosis factor-α(TNF-α),white blood cell count(WBC),C-reactive albumin(CRP)],the levels of immune function indexes(CD^(+)_(3),CD^(+)_(4),CD^(+)_(4)/CD^(+)_(8)),Th1/Th2 cytokines[interferon-γ(IFN-γ),interleukin-4(IL-4)] before and after treatment of the two groups were compared.Results The total effective rate(92.73%,51/55) of the combination group after treatment was higher than that of the control group(78.18%,43/55)(P<0.05).The scores of pustules,acne,inflammatory papules and nodular cysts and DLQI scores of the combination group after treatment were lower than those of the control group(P<0.05).The serum levels of TNF-α,WBC,CRP and IL-4 in the combination group were lower than those in the control group,and the levels of IFN-γ,CD^(+)_(3),CD^(+)_(4) and CD^(+)_(4)/CD^(+)_(8) were higher than those in the control group(P<0.05).Conclusion The combination of Modified Xiaoyao Powder and pulsed dye laser in the treatment of moderate and severe acne can effectively regulate cellular immunity and humoral immunity,thereby reducing the immune inflammatory response,effectively alleviating the symptoms of acne and improving the quality of life of patients.The effect is remarkable.
