夏枯草汤加减结合雷火神针治疗良性甲状腺结节(气郁痰阻型)临床效果观察

摘　　要：目的 探究夏枯草汤加减结合雷火神针治疗良性甲状腺结节(气郁痰阻型)临床效果。方法 研究纳入99例良性甲状腺结节(气郁痰阻型)患者(2019年7月—2020年5月),采取随机数字表法将99例患者分成两组,分别为对照组50例与观察组49例,对照组口服优甲乐3个月治疗,观察组在对照组治疗基础上+夏枯草汤加减结合雷火神针治疗,治疗3个月,比较各组患者疗效、治疗前后患者甲状腺结节数目及甲状腺结节大小变化、中医证候积分(颈部触及结节、喉间梗阻感、善太息、情志抑郁、头晕目眩等)变化、甲状腺功能指标变化及血清中胰岛素样生长因子-1(IGF-1)、转化生长因子-β(TGF-β)及血管内皮生长因子(VEGF)水平变化、结节内血流信号分级情况变化、不良反应。结果 与观察组(95.92%,47/49)相比,对照组患者有效率(84.00%,42/50)较低(P<0.05);治疗前,两组患者甲状腺结节数目及甲状腺结节大小、中医证候积分(颈部触及结节、喉间梗阻感、善太息、情志抑郁、头晕目眩等)、IGF-1、TGF-β、VEGF、结节内血流信号分级情况比较(P>0.05),治疗后各组患者甲状腺结节数目及甲状腺结节大小、中医证候积分(颈部触及结节、喉间梗阻感、善太息、情志抑郁、头晕目眩等)、IGF-1、TGF-β、VEGF、结节内血流信号分级情况均改善,观察组患者治疗后甲状腺结节数目及甲状腺结节大小、中医证候积分(颈部触及结节、喉间梗阻感、善太息、情志抑郁、头晕目眩等)、IGF-1、TGF-β、VEGF、结节内血流信号分级情况等指标优于对照组患者(P<0.05);两组患者治疗前后促甲状腺激素(TSH)、游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)等甲状腺功能指标无变化(P>0.05);对照组失眠1例,口干1例,观察组口干1例,不良反应比较,P>0.05。结论 夏枯草汤加减结合雷火神针治疗良性甲状腺结节(气郁痰阻型)临床效果佳,安全可靠,值Objective To explore the clinical effect of Xiucao Decoction(夏枯草汤) combined with thunder-fire miraculous needle in the treatment of benign thyroid nodule(Qi stagnation and phlegm obstruction type).Methods A total of 99 patients with benign thyroid nodules(Qi stagnation and phlegm obstruction type) were included in this study(July 2019 to May 2020).The patients were divided into two groups by random number table method:control group(50 cases) and observation group(49 cases).The control group received Euthyrox orally for 3 months.The observation group was additionally treated with Xiakucao Decoction and thunder-fire miraculous needle for 3 months.The curative effect,the number of thyroid nodules and the size of thyroid nodules before and after treatment,the changes of traditional Chinese medicine syndrome scores(cervical touching nodules,feeling of laryngeal obstruction,deep sighing,emotional depression,dizziness,etc.),the changes of thyroid function indexes,the levels of insulin-like growth factor-1(IGF-1),transforming growth factor-1β(TGF-1β) and vascular endothelial growth factor(VEGF) in serum,the grading of blood flow signals in nodules,and adverse reactions were compared.Results Compared with that(95.92%,47/49) of the observation group,the effective rate(84.00%,42/50) of the control group was lower(P0.05).After treatment,the number and size of thyroid nodules,traditional Chinese medicine syndrome scores(cervical touching nodules,feeling of laryngeal obstruction,deep sighing,emotional depression,dizziness,etc.),levels of IGF-1,TGF-1β and VEGF,and grade of blood flow signal in nodules were all improved in each group.Those in the observation group were better than those in the control group(P0.05).There was 1 case of insomnia and 1 case of dry mouth in the control group and 1 case of dry mouth in the observation group.The adverse reactions were not significant(P>0.05).Conclusion Xiakucao Decoction combined with thunder-fire miraculous needle has good clinical effect in the treatment of benign thyroid
