摘　　要：目的:探讨专病一体化管理对肺癌患者化疗间歇期症状群迁移的影响。方法:选取2019年1月1日~12月31日呼吸与危重症医学科行化疗的60例肺癌患者作为研究对象,随机分为对照组和观察组各30例,对照组采用常规健康教育,观察组在对照组基础上采用专病一体化管理;比较首次化疗7 d后、干预6个月后两组症状严重程度与困扰[采用安德森症状评估量表中文版(MDASI-C)],采用探索性因子分析提取首次化疗7 d后、干预6个月后观察组症状群。结果:干预6个月后,观察组MDASI-C中苦恼、食欲下降、疲乏、睡眠不安、昏昏欲睡、悲伤感、咳嗽、便秘、一般活动、生活乐趣、情绪得分均低于对照组(P<0.05,P<0.01);干预6个月后,观察组症状群提取出5个公因子,分别为疲乏、气短、食欲下降、呕吐、苦恼、麻木感、悲伤感、便秘、咽喉痛、气短。结论:专病一体化管理可缓解肺癌患者化疗相关症状,减轻疾病困扰,改善生活质量。Objective:To explore the effect of professional integration management on the symptoms of lung cancer patients during the interval of chemotherapy.Methods:A total of 60 lung cancer patients admitted during January 1 to December 31,2019 were selected as the study objects,and were randomly divided into the control group and the observation group,with 30 cases each.The control group received conventional health education.The observation group received professional integration management on the basis of the control group.The severity of symptoms and disturbance to life by MDASI-C were compared between the two groups 7 d and 6 months after intervention.Using exploratory factor analysis to extract the symptoms in the observation group 7 d after the first chemotherapy and 6 months after the intervention.Results:Six months after intervention,the scores of distress,loss of appetite,fatigue,sleep disturbance,drowsiness,sadness,cough,constipation,general activities,life pleasure and emotions of MDASI-C were in the observation group than those in the control group(P<0.05,P<0.01).Six months after intervention,5 common factors were extracted from the symptoms in the observation group,which were respectively fatigue,shortness of breath,loss of appetite,vomiting,pain,numbness,sadness,constipation,sore throat and shortness of breath.Conclusion:The professional integration management can reduce chemotherapy related symptoms,reduce the influence on life,and improve the quality of life in lung cancer patients.
