摘 要:目的 探讨特发性肺纤维化(IPF)患者支气管肺泡灌洗液(BALF)及血清中可溶性细胞粘附分子-1(sICAM-1)、基质金属蛋白酶-2(MMP-2)的水平及其临床意义。方法 选取我院收治的IPF患者118例为IPF组,同期健康体检者63例为对照组。采用酶联免疫吸附法(ELISA)测定两组BALF和血清中sICAM-1和MMP-2水平,分析IPF的相关影响因素;采用GAP分期标准对IPF患者进行疾病严重程度分期,Ⅰ期38例,Ⅱ期47例,Ⅲ期33例,比较不同疾病分期患者BALF和血清中sICAM-1、MMP-2水平、胸部高分辨CT(HRCT)评分差异,分析IPF患者BALF与血清中sICAM-1和MMP-2水平与其胸部HRCT评分的关系。结果 与对照组相比,IPF组BALF及血清中sICAM-1、MMP-2表达水平均升高(P<0.05);年龄≥60岁、有吸烟史、BALF和血清中sICAM-1、MMP-2过度表达是IPF发生的独立危险因素(P<0.05);与Ⅰ期、Ⅱ期IPF患者相比,Ⅲ期IPF患者BALF和血清中sICAM-1、MMP-2表达均升高(P<0.05);IPF患者BALF和血清中sICAM-1、MMP-2水平分别与其胸部HRCT评分呈正相关(P<0.05)。结论 IPF患者BALF和血清中sICAM-1、MMP-2水平均增高,且与患者肺功能有一定关联性,可作为IPF患者病情监测的评价指标。Objective To investigate the expression levels of soluble intercellular adhesion molecule-1(sICAM-1) and matrix metalloproteinase-2(MMP-2) in bronchoalveolar lavage fluid(BALF) and serum of patients with idiopathic pulmonary fibrosis(IPF), and their clinical significance.Methods This study included 118 patients with IPF admitted to our hospital(IPF group) and 63 healthy individuals(control group). The levels of sICAM-1 and MMP-2 in BALF and serum of the two groups were determined by enzyme-linked immunosorbent assay(ELISA). The influencing factors of IPF were analyzed. The GAP staging standard was used to classify the severity of IPF. The results showed that there were 38 cases in stage I, 47 in stage II and 33 in stage III. The levels of sICAM-1 and MMP-2 in BALF and serum, and the chest high-resolution computed tomography(HRCT) score were compared among different stages. The relationship between the levels of sICAM-1 and MMP-2 in BALF and serum and the chest HRCT score in patients with IPF was analyzed.Results Compared with the control group, the levels of sICAM-1 and MMP-2 in BALF and serum in the IPF group were increased(P<0.05). Logistic regression analysis showed that age ≥ 60, smoking history, and overexpression of sICAM-1 and MMP-2 in BALF and serum were independent risk factors for IPF(P<0.05). Compared with patients with IPF in stage I and stage II, patients with IPF in stage III had higher levels of sICAM-1 and MMP-2 in BALF and serum(P<0.05). The levels of sICAM-1 and MMP-2 in BALF and serum of patients with IPF were positively correlated with chest HRCT score(P<0.05).Conclusion Patients with IPF have high levels of sICAM-1 and MMP-2 in BALF and serum that are related to the patients′ lung function. The two indicators can be used for disease condition monitoring of patients with IPF.
