摘 要:目的:了解中国援非医疗队员心理压力与职业倦怠状况及其相关因素。方法:采用自制的一般情况调查表、中文版知觉压力量表和Maslach职业倦怠量表对2021年10月5日—10月20日期间在非洲执行援外医疗任务的210名队员进行调查,采用Logistic回归分析医疗队员心理知觉压力和职业倦怠的相关因素。结果:38.6%(81/210)的队员存在心理压力,17.5%(37/210)的队员存在轻中度职业倦怠,9.9%(21/210)的队员存在中重度职业倦怠。Logistic回归分析结果显示,对驻地安全不满意(OR=3.367,95%CI:1.498~7.570)和当地医院不重视(OR=3.751,95%CI:1.684~8.353)是心理知觉压力可能的危险因素,家庭顺心(OR=0.439,95%CI:0.003~57.419)是心理知觉压力保护因素。初级职称(OR=2.951,95%CI:1.443~6.034)和对驻地安全性不满意(OR=2.625,95%CI:1.210~5.964)是职业倦怠可能的危险因素,家庭顺心是职业倦怠的保护因素(OR=0.010,95%CI:0.000~0.742)。结论:援非医疗队员存在一定程度的心理知觉压力和职业倦怠,初级职称、对驻地安全性不满意和当地医院不重视是可能的危险因素,家庭顺心是保护因素,应开展针对性的措施改善医疗队员的心理健康状况和降低职业倦怠。Objective: To investigate the psychological stress and burnout of Chinese medical team members in Africa and their related factors. Methods: A self-administered general condition questionnaire, a Chinese version of the Perceived Stress Scale and the Maslach Burnout Scale were used to survey 210 medical team members who were on foreign medical missions in Africa between October 5 and October 20, 2021, and logistic regression was used to analyze the factors associated with psychological stress and burnout among medical team members.Results: 38.6%(81/210) of the team members had psychological stress, 17.5%(37/210) had mild to moderate burnout, and 9.9%(21/210) had moderate to severe burnout. Logistic regression analysis showed that dissatisfaction with resident safety(OR=3.367, 95% CI: 1.498-7.570) and lack of attention from the local hospitals(OR=3.751, 95% CI: 1.684-8.353) were possible risk factors for psychological stress, and family satisfaction(OR=0.439, 95% CI: 0.003-57.419) was a protective factor for psychological stress. Junior title(OR=2.951, 95% CI: 1.443-6.034) and dissatisfaction with resident safety(OR=2.625, 95% CI: 1.210-5.964) were possible risk factors for burnout, and family satisfaction was a protective factor for burnout(OR=0.010, 95% CI:0.000-0.742). Conclusion: There is a certain degree of psychological pressure and burnout among the Chinese medical team members in Africa. The Junior title, dissatisfaction with the safety of the resident and lack of attention from local hospitals are possible risk factors. Family satisfaction is a protective factor. Targeted measures should be carried out to improve psychological health of medical team members and reduce burnout.
