摘  要:目的应用CT能谱成像多参数评估大鼠C6脑胶质瘤图像引导放疗(IGRT)后肿瘤细胞的增殖状态和脑组织含水量变化。方法建立48只雄性Wister大鼠C6脑胶质瘤模型。随机分为2组:对照组和IGRT组,每组各24只,荷瘤鼠前期预试验生存分析每组各6只。所有大鼠接种C6胶质瘤细胞后分别于第14、20、26天扫描CT能谱成像,每次扫描完成后每组随机处死3只测定脑组织含水量、3只组织病理学分析。结果对照组和IGRT组荷瘤鼠生存期差异无统计学意义(P>0.05)。与对照组相比,IGRT组荷瘤鼠脑组织含水量和CT能谱成像瘤周区水质量浓度随时间呈明显升高趋势,且瘤周区水质量浓度与荷瘤鼠脑组织含水量具有较强的相关性。IGRT组荷瘤鼠肿瘤实体区Ki-67增殖指数和CT能谱成像碘质量浓度、能谱曲线斜率呈明显的降低趋势,且Ki-67增殖指数与碘质量浓度具有较强的相关性。结论CT能谱成像碘质量浓度、水质量浓度和能谱曲线斜率能够早期动态监测C6胶质瘤IGRT治疗后肿瘤Ki-67增殖指数的状态,且能够无创性监测放射性脑水肿的变化。Objective To conduct a multiparametric evaluation of spectral CT imaging in rat C6 glioma after image-guided radiotherapy(IGRT)for tumor cell proliferation status and changes in brain water content.Methods 48 male Wister rats C6 glioma models were established and randomly divided into 2 groups:the control group and the IGRT group,with 24 rats in each group and a 6 rats/group for pre-experimental survival analysis of tumor-bearing rats.All rats were scanned for CT spectral imaging on the 14th,20th,and 26th days after inoculation with C6 glioma cells.After each CT scan,3 rats/group were randomly sacrificed to determine the water content of brain tissue,and 3 rats/group were analyzed for histopathology.Results There was no significant difference in the survival time between the control group and the IGRT group(P>0.05).Compared with the control group,the water content in the brain tissue of the tumor-bearing rats and the water concentration in the peritumoral area of the tumor-bearing rats in the IGRT group increased significantly with time.There was a strong correlation between the water concentration in the peritumoral area and that in the brain tissue of tumor-bearing rats.In the IGRT group,the Ki-67 proliferation index in the tumor solid area of the tumor-bearing rats,the iodine concentration of CT energy spectrum imaging and the slope of the energy spectrum curve(λ)showed a significant decreasing trend.The Ki-67 proliferation index had a strong correlation with the iodine concentration.Conclusion Spectral CT imaging of iodine concentration,water concentration andλcould dynamically monitor the status of tumor Ki-67 proliferation index after IGRT treatment of C6 glioma,and could non-invasively monitor the changes of radiation-induced brain edema.
